感兴趣区的描绘对新辅助化疗乳腺癌超声造影定量参数的影响
Effect of Region of Interest Characterization on Quantitative Parameters of Contrast-enhanced Ultrasound in Breast Cancer with Neoadjuvant Chemotherapy
【摘要】 目的 探讨感兴趣区(ROI)的描绘对新辅助化疗(NAC)乳腺癌超声造影(CEUS)定量参数的影响。方法 收集NAC前后均行CEUS的乳腺癌患者58例。采用动态对比时间-强度曲线(TIC)波峰法及目测法确定病灶增强较明显区域放置ROI获取各定量参数。改变ROI及观察者,根据ROI大小分2 mm、4 mm、6 mm组,通过组内相关系数(ICC)评估观察者间定量参数一致性,比较不同ROI间定量参数变化率的差异。结果 两种方法观察者间的强度参数ICC>0.75;时间参数中,除目测法4 mm、6 mm组及动态对比TIC波峰法6 mm组的时间参数-达峰时间(TTP)的ICC>0.75,其余参数ICC<0.75。NAC前后不同ROI各定量参数变化率均无统计学差异(P>0.05)。结论 强度参数稳定性好,可用于预测或评估乳腺癌NAC疗效;乳腺癌NAC前后应保持ROI大小一致;2 mm的ROI更适用于疗效评估。
【Abstract】 Objective To explore the effect of region of delineation of region of interest(ROI) on quantitative parameters of contrast-enhanced ultrasound(CEUS) in breast cancer with neoadjuvant chemotherapy(NAC). Methods A total of 58 breast cancer patients who underwent CEUS before and after NAC were collected. The dynamic comparison time-intensity curve(TIC) crest method and visual method were used to determine the ROI with obvious enhancement of lesions to obtain the quantitative parameters. Then we changed the size of ROI and observer, and divided the patients into 2 mm group, 4 mm group, and 6 mm group according to the size of the ROI. The quantitative parameters between observers were evaluated by intra-group correlation coefficient(ICC). The change rates of quantitative parameters before and after NAC were compared among different ROIs. Results The intensity parameter ICC between the two methods was more than 0.75. In terms of time parameters, except for the 4mm and 6mm groups by visual inspection and the 6 mm group by dynamic comparison with TIC crest method, the ICC of TTP ICC was >0.75, and the other parameters were <0.75. Before and after NAC, there was no difference in the change rate of each quantitative parameter in different ROI(P>0.05). Conclusions The intensity parameters are stable and can be used to predict or evaluate the curative effect of NAC in breast cancer. The ROI of breast cancer should be consistent before and after NAC. 2 mm ROI is more suitable for curative effect evaluation.
