超声剪切波弹性成像评价亚急性期下肢深静脉血栓弹性硬度的价值
The Value of Ultrasonic Shear Wave Elastography in Evaluating the Elastic Hardness of Subacute Lower Extremity Deep Vein Thrombosis
【摘要】 目的 探讨超声剪切波弹性成像(SWE)评价亚急性期下肢深静脉血栓(DVT)弹性硬度的临床价值。方法 纳入亚急性期(15~30 d)下肢DVT且完全闭塞的目标血管144段为研究对象。应用二维超声评估目标血栓回声强度并评分,SWE测量血栓的最小弹性模量值(E-min)、最大弹性模量值(E-max)、平均弹性模量值(E-mean)和离散度(SD)。构建受试者工作特征曲线(ROC曲线),评价二维超声和SWE对亚急性期DVT的诊断效能。结果 (1)组间E-mean差异无统计学意义(P>0.05)。(2)血栓二维超声评分与E-Mean两两比较及ROC曲线分析,两种方法诊断亚急性期DVT的差异有统计学意义(P<0.05)。结论 E-mean可用于评价亚急性期血栓的弹性硬度,SWE可为临床DVT治疗时机和选择方案提供依据。
【Abstract】 Objective To explore the clinical value of ultrasonic shear wave elastography(SWE) in evaluating the elastic hardness of subacute lower extremity deep vein thrombosis(DVT). Methods 144 segment vessels of lower extremity deep vein diagnosed as total occlusion by color Doppler ultrasound(CDUS) in subacute period(15~30 d) were selected as case group. The echo intensity and score of thrombus were evaluated by two-dimensional(2D) ultrasound, and the minimum elastic modulus(E-min), the maximum elastic modulus(E-max), the mean elastic modulus(E-mean) and dispersion(SD) were measured by SWE. Establish ROC curve to evaluate the diagnostic effect of 2D ultrasound and SWE for subacute DVT. Results(1)Compared with E-mean among the groups, there was no significant difference(P>0.05).(2)With 2D ultrasound score of thrombus, pair-wise comparisons of E-mean and ROC curve analysis, there were significant difference between two the group in diagnosing subacute DVT(P<0.05). Conclusions E-mean could be used to evaluate the elasticity of thrombus in subacute DVT, and SWE could provide evidence to the treatment opportunity and plan choosing for DVT.
