超声和FNA-Tg联合在甲状腺乳头状癌颈部淋巴结转移的应用价值
Application Value of Ultrasound Combined with FNA-Tg in Cervical Lymph Node Metastasis of Papillary Thyroid Carcinoma
【摘要】 目的 分析甲状腺乳头状癌(PTC)患者颈部淋巴结超声特征评分和穿刺洗脱液甲状腺球蛋白(FNA-Tg)结果,探讨两者在PTC颈部淋巴结转移预测价值。方法 回顾性分析PTC根治术患者颈部可疑淋巴结超声特征评分及FNA-Tg结果。绘制ROC曲线提示超声评分>2分为最佳诊断阈值,采用Kappa检验分别比较超声评分>2、FNA-Tg>1 ng/mL与病理一致性;以ROC曲线比较超声评分、FNA-Tg及两者联合诊断效能。结果 超声评分、FNA-Tg与病理的一致性Kappa值分别为0.527、0.572;通过ROC曲线比较超声、FNA-Tg及两者联合,联合后提高了诊断的灵敏度和阴性预测值(P<0.05)。结论 超声评分及FNA-Tg对判断PTC颈部淋巴结转移均是较好的诊断方法,二者联合能提高诊断的灵敏度和阴性预测值。
【Abstract】 Objective To analyze the ultrasonographic characteristics of cervical lymph nodes and fine needle aspiration-thyroglobulin(FNA-Tg) in patients with papillary thyroid carcinoma(PTC) and to investigate the predictive value of both methods in cervical lymph node metastasis of PTC. Methods Ultrasonic characteristic scores and FNA-Tg of suspicious cervical lymph nodes in patients undergoing PTC radical operation were retrospectively analyzed. ROC curve was drawn to know that ultrasonic score >2 was classified as the optimal diagnostic threshold. Kappa test was used with ultrasonic score >2 and FNA-Tg>1 ng/mL as positive criteria and pathological consistency. ROC curve was used to compare the diagnostic efficiency of ultrasound score, FNA-Tg and their combination. Results The Kappa values of ultrasound score, FNA-Tg and pathology were 0.527 and 0.572, respectively. ROC curve was used to compare ultrasound, FNA-Tg and the combination of the two methods, which improved the diagnostic sensitivity and negative predictive value(P<0.05). Conclusions Both ultrasound score and FNA-Tg are good diagnostic methods for PTC cervical lymph node metastasis, and their combination can improve the diagnostic sensitivity and negative predictive value.
