超声引导下肘管综合征的介入治疗研究

【摘要】 目的 评估实时超声引导下尺神经周围药物注射及去除病因治疗肘管综合征的临床价值。方法 回顾性分析行超声引导下单侧尺神经周围药物注射治疗肘管综合征的患者40例,其中男23例,女17例,年龄42~68岁,平均(56±10.5)岁。结合临床病史及超声检查,对其中12例类风湿性关节炎患者、5例肘管处腱鞘囊肿患者进行针对病因的治疗。采用门诊和电话方式随访,分别记录治疗前和治疗后的患者尺侧手指疼痛的疼痛视觉模拟评分(VAS),应用配对t检验比较治疗前和治疗后1周、1个月的VAS评分。结果 40例患者肘管处尺神经平均最大横截面积约9.2 mm~2,其中12例类风湿患者可见肘关节滑膜增生挤压尺神经,5例患者可见肘管处腱鞘囊肿压迫尺神经。40例患者均成功进行超声引导下介入治疗。40例患者治疗前疼痛VAS评分(7.7±0.9)分,治疗后1周VAS评分(3.6±1.2)分,治疗后1个月VAS评分(3.1±1.1)分,与治疗前比较均明显降低并具有统计学意义(P<0.05)。结论 实时超声引导下尺神经周围药物注射及去除病因治疗肘管综合征是一种安全、有效的方法。

【Abstract】 Objective To evaluate the clinical value of ultrasound-guided drug injection around ulnar nerve and etiological treatment in the treatment of cubital tunnel syndrome. Methods 40 patients with cubital tunnel syndrome who received ultrasound-guided unilateral ulnar nerve injection in our hospital were reviewed. There were 23 males and 17 females, with an average age of(56 ±10.5) years old. Combined with clinical history and ultrasound examination, 12 patients with rheumatoid arthritis and 5 patients with ganglion cyst were treated according to the etiology. The visual analogue scale(VAS)score were recorded before and after treatment respectively. And the VAS scores before, 1 week and 1 month after treatment were compared by paired t-test. Results The average maximum cross-sectional area of ulnar nerve in the cubital canal of 40 patients was about 9.2 mm~2, in which 12 patients with rheumatoid arthritis could see elbow synovial hyperplasia squeezing ulnar nerve, 5 patients could see ganglion cyst compressing ulnar nerve. All 40 patients were successfully treated with ultrasound-guided interventional therapy. Before treatment, the pain VAS score of 40 patients was(7.7±0.9); at 1 week after treatment, the VAS score was(3.6±1.2); then at 1 month after treatment, it was(3.1±1.1), which were significantly lower than those before treatment(P<0.05). Conclusions Real time ultrasound-guided ulnar nerve drug injection and etiological treatment of cubital tunnel syndrome is a safe and effective method.
