黄芩苷改善肝纤维化动物模型的系统评价与Meta分析
Systematic Review and Meta-Analysis of Baicalin in Improving Animal Models of Liver Fibrosis
【摘要】 目的:采用Meta分析方法,系统评价黄芩苷干预肝纤维化动物模型的疗效。方法:计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed和the Cochrane Library中关于黄芩苷治疗肝纤维化的动物实验[实验组采用黄芩苷干预;对照组采用0.9%氯化钠溶液或阳性药物(如水飞蓟素等)干预],检索时限均为建库至2022年11月18日。根据纳入、排除标准筛选文献,提取资料并采用SYRCLE动物实验偏倚风险评估工具对纳入研究进行质量评估,使用RevMan 5.4软件进行Meta分析。结果:最终纳入9项研究。Meta分析结果显示,在改善肝纤维化方面,黄芩苷组动物模型的透明质酸酶、Ⅳ型胶原、层粘连蛋白、Ⅲ型前胶原、羟脯氨酸和转化生长因子β1水平显著低于对照组,差异均有统计学意义(P<0.01);黄芩苷组动物模型的肿瘤坏死因子α水平与对照组相比有明显降低趋势,但差异无统计学意义(P>0.05);在改善肝功能方面,黄芩苷组动物模型的丙氨酸转氨酶、天冬氨酸转氨酶和白蛋白水平显著低于对照组,差异均有统计学意义(P<0.05),但碱性磷酸酶水平与对照组的差异无统计学意义(P>0.05)。结论:黄芩苷对肝纤维化模型动物的肝纤维化相关指标和肝功能相关指标水平有明显的改善作用,该结果可为黄芩苷的临床研究提供支撑。
【Abstract】 OBJECTIVE: To systematically evaluate the efficacy of baicalin on animals with liver fibrosis models by using Meta-analysis. METHODS: CNKI, Wanfang Data, VIP, CBM, PubMed and the Cochrane Library were retrieved to collect the animal experiments on baicalin in the treatment of liver fibrosis(the experimental group was intervened with baicalin; the control group was intervened with 0.9% sodium chloride solution or positive drugs, such as silymarin). The retrieval time was from the establishment of the database to Nov. 18th, 2022. According to inclusion and exclusion criteria, data were extracted and quality assessment of included studies was performed by using the SYRCLE animal experiment bias risk assessment tool. RevMan 5.4 software was used for Meta-analysis. RESULTS: A total of 9 studies were ultimately included. Meta-analysis results showed that in terms of improving liver fibrosis, the levels of hyaluronidase, type Ⅳ collagen, laminin, type Ⅲ procollagen, hydroxyproline, and transforming growth factor β1 in the animal models of baicalin group were significantly lower than those in the control group, with statistically significant differences(P<0.01); the level of tumor necrosis factor α in the animal models of baicalin group was significantly lower than that in control group, the difference was not statistically significant(P>0.05). In terms of improving liver function, alanine aminotransferase, aspartate aminotransferase and albumin levels in the animal models of baicalin group were significantly lower than those in the control group, with statistically significant differences(P<0.05); yet alkaline phosphatase levels were not significantly different compared with the control group(P>0.05). CONCLUSIONS: Baicalin shows significant improvement in the levels of liver fibrosis-related indexes and liver function-related indexes in liver fibrosis model animals, and the results can provide support for the clinical study of baicalin.
