环泊酚应用于无痛胃镜检查中的效果及安全性研究

【摘要】 目的 探究环泊酚和丙泊酚在无痛胃镜检查中的镇静效果及安全性。方法 选取2022年12月至2023年3月在我院行无痛胃镜检查的患者104例,按随机数表法分为环泊酚组和丙泊酚组各52例,静脉注射瑞芬太尼0.3μg/kg后,两组分别静脉推注环泊酚0.4 mg/kg、丙泊酚1.5 mg/kg,比较两组镇静效果及恢复情况,入室时(T0)、意识消失时(T1)、置入胃镜时(T2)、操作结束时(T3)平均动脉压(MAP)、心率(HR)、血氧饱和度(SpO2),检查前、苏醒时、苏醒后30 min简易精神状态检查评分(MMSE)以及不良反应发生情况。结果 104例患者均成功镇静。环泊酚组患者检查时间短于丙泊酚组(P<0.05),两组苏醒时间和定向力恢复时间比较,差异无统计学意义(P>0.05);T1、T2时,两组MAP、HR均低于T0,且T2时点低于T1时点,环泊酚组T1、T2时MAP高于丙泊酚组(P<0.05);T3时,两组间差异无统计学意义(P>0.05)。两组苏醒时及苏醒后30 min的MMSE评分均低于检查前,苏醒后30 min高于苏醒时(P<0.05),组间比较差异无统计学意义(P>0.05);环泊酚组注射痛和不良反应总发生率均低于丙泊酚组(P<0.05)。结论 环泊酚与丙泊酚在无痛胃镜检查中的镇静效果相当,但环泊酚对患者血压波动的影响更小,且不良反应发生率较低,值得推广应用。

【Abstract】 Objective To explore the sedative effect and safety of ciprofol and propofol in painless gastroscopy.Methods One hundred and four patients who underwent painless gastroscopy in our hospital from December 2022 to March 2023 were selected. The patients were divided into a ciprofol group and a propofol group by means of the random number table method, 52 in each group. After intravenous injection of 0.3 μg/kg of remifentanill, the two groups were intravenously injected with 0.4 mg/kg of ciprofol and 1.5 mg/kg of propofol, respectively. The differences in sedative effect and recovery, mean arterial pressure(MAP), heart rate(HR) and oxygen saturation(SpO2) at the time of entry(T0), the time of loss of consciousness(T1), the time of gastroscope implantation(T2) and at the time of the end of operation(T3), Mini-Mental State Examination(MMSE) score before examination, at the time of awakening and at 30 min after awakening as well as occurrence of adverse reactions were compared between both groups.Results All 104 patients were sedated successfully. The examination time in the ciprofol group was shorter than that in the propofol group(P<0.05). However, there were no statistical differences in the awakening time and orientation recovery time between the two groups(P>0.05). At T1 and T2, MAP and HR of the two groups were lower than those at T0, and MAP and HR at T2 was lower than those at T1. The MAP of the cypofol group at T1 and T2 was higher than that of the propofol group(P<0.05). At T3, there was no significant difference between the two groups(P>0.05). The MMSE scores of the two groups at the time of recovery and after 30 min of recovery were lower than those before examination, and higher at 30 min after recovery than that at the time of recovery(P<0.05), but there was no statistical significance between the two groups(P>0.05). The incidence rate of injection pain and total incidence of adverse reactions were lower in the ciprofol group than those in the propofol group(P<0.05).Conclusions The sedative effect of ciprofol is similar to that of propofol in painless gastroscopy. However, ciprofol has smaller influence on blood pressure fluctuations of the patients and lower incidence rates of adverse reactions. It is worth promoting and applying.
