【摘要】 目的 探讨利用流程优化方法，优化急性缺血性脑卒中（Acute Ischemic Stroke,AIS）患者就医流程管理，提高患者就医效率。方法 通过六西格玛DMAIC模型并结合ESIA法，对目标医院AIS患者急诊就医流程进行院前—院内急救一体化优化，并对优化前后的关键指标进行评价。结果 AIS患者急诊就医流程优化后，就诊环节减少，六西格玛水平提高1.03δ，入院至接受溶栓治疗时间（Door-To-Needle Time,DNT）显著减少至22 min，在45 min内的比例提高至94.52%，患者费用未发生显著变化。结论 使用DMAIC模型和ESIA法能持续有效优化患者就医流程，有利于患者救治。更多还原

【Abstract】 Objective To explore the use of process optimization methods to optimize the process management of patients with acute ischemic stroke and improve the efficiency of patients’ medical treatment. Methods Six Sigma DMAIC model combined with ESIA method was used to optimize the pre-hospital and in-hospital emergency treatment process of AIS patients in the target hospital, and the key indicators before and after optimization were evaluated. Results After the optimization of the emergency treatment process of AIS patients,the number of visits decreased,the level of Six Sigma increased by 1.03δ,the DNT was significantly reduced to 22 minutes,and the proportion within 45 minutes increased to 94.52%,and the patient cost did not change significantly. Conclusion The use of DMAIC model and ESIA method can continuously and effectively optimize the process of patient treatment.更多还原
