上海市女性居民宫颈癌认知和筛查参与现状及其影响因素
Status Quo and Influencing Factors of Cognition and Screening Participation of Cervical Cancer Among Female Residents in Shanghai
【摘要】 目的:分析上海市女性居民宫颈癌认知状况、筛查参与情况及其影响因素,为提高女性居民宫颈癌认知水平与筛查积极性提供参考。方法:采用自制宫颈癌防治知识认知问卷及筛查参与问卷对上海市487名女性居民进行调查。结果:女性居民宫颈癌防治知识得分为10(6,13)分。与未接触过防治知识的女性居民相比,了解1-2种防治知识的女性居民低认知的风险为0.542倍(P=0.046),了解3-6种防治知识的女性居民低认知的风险为0.162倍(P<0.001)。通过医疗相关途径可降低女性居民对宫颈癌危险因素、筛查方式、知识误区及一般知识的低认知水平风险(OR=0.538,P=0.011;OR=0.347,P<0.001;OR=0.441,P=0.001;OR=0.580,P=0.021)。认知总分每增加1分,女性居民不参加宫颈癌筛查的风险降低0.142倍(P<0.001)。结论:上海市女性居民宫颈癌防治认知整体水平相对较低,防治知识了解途径数的增加能提高认知水平,应通过多途径多维度提升女性居民认知水平,促进女性居民参与宫颈癌筛查。
【Abstract】 Objective: To analyze the status and influencing factors of cognition of cervical cancer and participation in screening among female residents in Shanghai, so as to provide references for improving cognition level and enthusiasm of screening among female residents. Methods: 487 female residents in Shanghai were investigated by self-made cervical cancer prevention knowledge awareness questionnaire and screening participation questionnaire. Results: The score of knowledge of prevention and treatment of cervical cancer of female residents was 10(6,13). Compared with female residents exposed to zero access to knowledge of prevention and treatment, the risk of low cognition was 0.542 times in 1-2 accesses(P=0.046), and 0.162 times in 3-6 accesses(P<0.001). Female residents who learned about risk factors of cervical cancer, screening methods, knowledge misunderstanding and general knowledge through medical approaches had significantly lower risk of low cognition(OR=0.538, P=0.011; OR=0.347, P<0.001; OR=0.441, P=0.001; OR=0.580, P=0.021). For every one-point increase in cognitive score, a female resident’s risk of not attending cervical cancer screening was 0.142-fold lower(P<0.001). Conclusion: The overall level of cognition of prevention and treatment of cervical cancer among female residents of Shanghai is relatively low, and the increase in the number of ways to know the cognition of prevention and treatment can improve the level of knowledge. Therefore, it is necessary to improve the cognition of female residents through multi-channels and multi-dimensions to promote the participation of female residents in cervical cancer screening.
