【摘要】 目的：探索建立临床药师参与的风湿免疫医药联合门诊服务新模式，降低风湿免疫疾病患者因复杂且长期用药带来的风险，提高患者用药正确率和依从性，促进合理用药。方法：本院基于风湿免疫科门诊，通过整合临床药师共同参与药学服务，完善软硬件设施及规章制度，建立了风湿免疫医药联合门诊，并开展服务。结果：经过风湿免疫医药联合门诊诊疗6个月后，115例类风湿关节炎（RA）患者的用药依从性、疾病活动度及血沉（ESR）、C-反应蛋白（CRP）指标均有显著改善（P <0.05）。结论：临床药师参与医药联合门诊实践，可提高药学服务质量，促进合理用药。更多还原

【Abstract】 Objective: In order to reduce the risk of complicated and long-term medication for patients with chronic rheumatic immune diseases, improve the accuracy and compliance of patients’ medication, and promote rational medication, a new model of joint outpatient service for rheumatic immune diseases with the participation of clinical pharmacists was established. Methods: Based on the clinic of rheumatic immune, our hospital established a physician-pharmacist joint clinic of rheumatic immune by integrating clinical pharmacists to provide pharmaceutical services, and improving soft/hardware facilities and regulations. Results: After 6 months treatment in the physician-pharmacist joint clinic of rheumatic immune, 115 patients with rheumatoid arthritis(RA) showed significant improvements in medication compliance, disease activity, erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP) level.Conclusion: The participation of clinical pharmacists in the practice of joint outpatient service can provide patients with high-quality pharmaceutical services and promote rational medication.更多还原
