宫颈癌组织程序性细胞死亡因子蛋白表达及与早期患者根治术后复发的关系
Expressions of PDCD4 protein in cervical cancer and its relationship with recurrence after radical operation
【摘要】 目的 检测宫颈癌组织中程序性细胞死亡因子4(PDCD4)蛋白表达,分析其与早期患者根治术后复发的关系。方法 选取2019年1月至2021年8月收治的102例拟行根治术的早期宫颈癌患者,术后取癌组织和边缘正常组织,均采用免疫组织化学二步法检测PDCD4蛋白表达。比较不同组织PDCD4蛋白阳性表达率;随访3年,比较复发与未复发患者癌组织PDCD4蛋白阳性表达率;利用Logistic回归分析明确癌组织PDCD4蛋白阳性表达与根治术后复发的关系。结果 癌组织和边缘正常组织PDCD4蛋白阳性表达率分别为34%(32/94)和76%(71/94),癌组织低于边缘正常组织,差异有统计学意义(χ~2=32.661,P<0.01);102例患者中8例失访,其余94例患者术后3年内复发率为37%。其中,复发和未复发患者PDCD4蛋白阳性表达占比分别为11%和47%,复发患者低于未复发患者,差异有统计学意义(χ~2=12.700,P<0.01);癌组织PDCD4蛋白阳性表达是复发的保护因素(OR=0.566,P=0.035);Ⅱa期、未/低分化、有脉管瘤栓、间质浸润深度>1/2、淋巴结阳性、切缘阳性均是根治术后复发的独立危险因素(OR=4.693、7.294、7.729,P=0.011、<0.01、0.001)。结论宫颈癌组织PDCD4蛋白阳性表达率低,且癌组织PDCD4蛋白阳性表达可降低早期患者根治术后复发风险。
【Abstract】 Objective To detect the expression of programmed cell death factor 4(PDCD4) protein in cervical cancer, and analyze the relationship between the recurrence and early patients after radical operation.Methods A total of 102 patients with early cervical cancer undergoing radical operation from January 2019 to August 2021 were selected. After surgery, the cancer tissue and marginal normal tissue were collected. The expres-sions of PDCD4 protein in tumor tissues and marginal normal tissues were detected by immunohistochemical two-step method. The positive expression rates of PDCD4 protein in different tissues were compared. After 3 years of following-up, the positive expression rates of PDCD4 protein were compared between recurrent and non recurrent patients. Logistic regression analysis was used to determine the relationship between PDCD4 protein expression and recurrence after radical operation. Results The positive expression rates of PDCD4 protein in cancer tissue and marginal normal tissue were 34%(32/94) and 76%(71/94), respectively. The difference between cancer tissue and marginal normal tissue was statistically significant( χ~2=32.661, P<0.01); Among the 102 patients, 8 were lost to follow-up, and the recurrence rate of 94 patients within 3 years after surgery was 37%. Among them, the positive expression rates of PDCD1 protein in cancer tissues of recurrent and non recurrent patients were 11% and47%, respectively. The positive expression rate of PDCD1 protein in recurrent patients was lower than that in non recurrent patients, with a statistically significant difference( χ~2=12.700, P<0.01); The positive expression rates of PDCD4 protein in recurrent and non recurrent patients were 11% and 47%, respectively. The positive expression of PDCD4 protein in cancer tissue was a protec-tive factor for recurrence(OR=0.566, P=0.035); Stage Ⅱa, undifferentiated/poorly differentiated, presence of vascular tumor thrombi, depth of interstitial in-filtration greater than 1/2, positive lymph nodes, and positive surgical margins were independent risk factors for postoperative recurrence after radical surgery(OR=4.693, 7.294, 7.729;P=0.011, <0.01, 0.001). Conclusion The positive expression rate of PDCD4 protein in cervical cancer tissue is low, and the positive expression of PDCD4 protein in cancer tissues can reduce the recurrence risk of early pa-tients after radical operation.
