基于功能MRI无先兆偏头痛患者动态功能连接研究

【摘要】 目的:探究无先兆偏头痛患者(MwoA)默认网络(DMN)与视觉网络(VN)功能连接的动态变化。方法:纳入45例MwoA患者和45例健康志愿者为研究对象,对所有被试者进行静息态功能磁共振(fMRI)检查。采用DPARSFA软件对fMRI数据进行预处理,采用GIFT软件对预处理MRI数据进行独立成分分析并挑选出34个独立成分作为内在连接网络,使用滑动时间窗技术分析得到4个功能连接状态(state)、动态功能连接(dFC)指标,结果经过FDR校正。采用Pearson相关分析计算MwoA组的dFC指标与临床资料的相关性。结果:MwoA组在state 1状态停留时间长。在state 2状态中,MwoA患者VN内功能连接强度减低,DMN与VN之间呈现弱连接状态(P<0.05)。dFC指标中的state 1持续时间与头痛影响测试-6分数具有正相关,转化次数与病程呈负相关。结论:基于独立成分分析的dFC方法探究到MwoA患者DMN与VN之间以长时间弱连接状态为主要异常表现,MwoA患者可能以视觉空间记忆障碍为早期的认知损伤。

【Abstract】 Purpose: To investigate the dynamic alterations in the functional link between the Default Mode Network(DMN) and Visual Network(VN) in patients with migraine without aura(MwoA). Methods: Forty-five MwoA patients and 45 healthy volunteers were included in the study, and all participants were undergone restingstate functional MRI(fMRI). DPARSFA software was used to preprocess the fMRI data. GIFT software was used to analyze the independent components of the preprocessed MRI data, and 34 independent components were selected as the intrinsic connectivity network. Four functional connection states and dynamic feature connectivity(dFC)indicators(time fraction, average dwell time and conversion times) were obtained by sliding time window analysis,and the results were FDR-corrected. Pearson correlation analysis was used to calculate the correlation between dFC index and clinical data in MwoA group.Results: The MwoA group holded the state 1 for a long time. In the MwoA group, the dFC within the VN, dFC between the VN and the DMN decreased in the state 2(P<0.05). The duration of state in the dFC was positively correlated with the headache impact test-6 scores, and the conversion times were negatively correlated with disease duration.Conclusion: The dFC method based on independent component analysis explored the long-term weak connection state between the DMN and VN in patients with MowA. Visual-spatial memory impairment may be an early cognitive impairment.
