99mTcO4-甲状腺显像评估分化型甲状腺癌术后残甲的价值分析

【摘要】 目的 探讨分化型甲状腺癌术后、131I治疗前99mTcO4-甲状腺显像评估残甲的临床应用价值。方法 选取我科收治的56例分化型甲状腺癌患者,测定甲状腺蛋白(Tg)、甲状腺球蛋白抗体(TgAb)、促甲状腺激素(TSH),行99mTcO4-甲状腺显像、131I显像。分析99mTcO4-甲状腺显像灵敏度、特异度、准确度、阳性预测值、阴性预测值、残甲灶检出率,残甲摄鍀计数与Tg/TSH相关性。结果 99mTcO4-甲状腺显像灵敏度86.79%,特异度66.67%,准确度85.71%,阳性预测值97.87%,阴性预测值22.22%,残甲灶检出率69.23%。残甲摄鍀计数与Tg/TSH显著相关(r=0.617,P<0.01)。结论分化型甲状腺癌术后、131I治疗前99mTcO4-甲状腺显像评估残甲临床应用价值较高,具有较高的灵敏度和阳性预测价值以及令人满意的残甲灶检出率。残甲摄鍀计数与Tg/TSH两者显著相关。

【Abstract】 Objective To explore the value of 99mTcO4-thyroid scan in detecting remnant thyroid tissue in thyroide ctomized differentiated thyroid cancer. Methods 56 patients with differentiated thyroid cancer admitted to our department were selected. Tg, TgAb and TSH were determined, and 99mTcO4- thyroid scan and 131I scan were performed. The sensitivity, specificity, accuracy, positive predictive value, negative predictive value and detection rate of remnant thyroid tissue, as well as the correlationbetween technetium residue count and Tg/TSH of 99mTcO4- thyroid scan were analyzed. Results The sensitivity, specificity, accuracy, positive predictive value, negative predictive value and detection rate of remnant thyroid tissue of 99mTcO4- thyroid scan were 86.79%, 66.67%, 85.71%, 97.87%, 22.22% and 69.23%, respectively. Technetium residue counts were significantly cor related with Tg/TSH. Conclusion 99mTcO4- thyroid scan after differentiated thyroid cancer surgery and before 131I treatment is ofhigh clinical application value in the evaluation of remnant thyroid tissue, with high sensitivity, positive prediction value and sat isfactory detection rate. There is a significant correlation between technetium count and Tg/TSH.
