【摘要】 目的 探讨MRI肝脏T₁弛豫时间在乙肝肝硬化门静脉高压患者中与肝静脉压力梯度（HVPG）相关性。方法 选取本院接治的92例乙肝肝硬化患者的临床资料，以HVPG值划分为高压组和对照组。其中以HVPG>5 mmHg为高压组（n=60）,以HVPG<5 mmHg为对照组（n=32）。分析两组一般临床资料、生化指标、凝血酶原时间、肝脏T₁弛豫时间、肝脏硬度检测（LSM）及增强肝纤维化（ELF）评分，分析肝硬化门静脉高压患者肝脏T₁弛豫时间与HVPG的关系。结果 高压组患者凝血酶原时间、肝脏T₁弛豫时间长于对照组，差异均有统计学意义（P<0.05）;此外，高压组患者LSM、ELF评分及HVPG均明显高于对照组，差异均有统计学意义（P<0.05）;高压组肝脏T₁弛豫时间与HVPG均呈正相关（r=0.533,P<0.05）。结论 乙肝肝硬化门静脉高压患者肝脏T₁弛豫时间联合其他指标可代替HVPG,用于无创检测乙肝肝硬化门静脉高压，适合临床推广。更多还原

【Abstract】 Objective To explore the correlation between relaxation time T₁ of MRI and hepatic venous pressure gradient（HVPG） in Hepatitis B cirrhosis patients with portal hypertension. Methods The clinical data of 92 patients with hepatitis B cirrhosis treated were retrospectively analyzed. They were divided into high pressure group and control group according to HVPG value. Among them, HVPG > 5 mmhg was the high-pressure group（n=60）, and HVPG < 5 mmhg was the control group（n=32）. The clinical data, biochemical indexes, prothrombin time, relaxation time T₁, liver stiffness measurement（LSM） and enhanced liver fibrosis（ELF） score were collected and analyzed. The relationship between relaxation time T₁ and HVPG was analyzed. Results Prothrombin time and relaxation time T₁ of liver in high-pressure group were significantly longer than those in control group（P<0.05）; In addition, the LSM, ELF score and HVPG of the high-pressure group were significantly higher than those of the control group（P<0.05）; There was a positive correlation between relaxation time T₁ and HVPG（r=0.533, P<0.05）. Conclusion Liver relaxation time T₁ combined with other indicators can replace HVPG in hepatitis B cirrhosis patients with portal hypertension. It is suitable for non-invasive detection of hepatitis B cirrhosis patients with portal hypertension and is suitable for clinical promotion.更多还原
