呼吸康复训练对支气管哮喘儿童肺功能及运动耐力的影响

【摘要】 目的 探讨呼吸康复训练对支气管哮喘患儿肺功能及运动耐力的影响。方法 选取2019年9月至2020年2月该院收治的46例支气管哮喘规范化治疗1年以上肺功能呈轻度阻塞性通气障碍的患儿。随机分为试验组(25例)及对照组(21例),对照组患儿严格按照全球哮喘防治创议(GINA)推荐的儿童哮喘药物治疗方案进行常规治疗,试验组患儿在对照组治疗基础上,结合个体情况进行为期3个月的呼吸康复训练。两组患儿均在干预前后进行肺功能、吸气肌力、6 min步行试验距离(6MWD)测量。结果 干预后,试验组患儿用力肺活量(FVC)88.91%±7.74%、1 s用力呼气量(FEV1) 89.96%±8.05%、FEV1/FVC 96.44±7.75,对照组患儿FVC 83.32%±8.21%、FEV1 84.20%±9.46%、FEV1/FVC 87.43±7.54,两组患儿肺功能各项指标较干预前均有一定提高,但试验组更为显著,且明显高于对照组(P<0.05);试验组患儿吸气肌力为(89.06±19.22)cm H2O(1 cm H2O=0.098 kPa),对照组患儿吸气肌力(71.56±12.84)cm H2O,试验组较对照组提高显著,差异有统计学意义(P<0.05),对照组较干预前无提高;试验组患儿6 min步行试验距离为(571.87±53.98)m,对照组患儿6 min步行试验距离为(536.43±47.59)m,两组患儿较干预前均有提高,但试验组更为显著,且明显高于对照组(P<0.05)。结论 呼吸康复训练能有效改善支气管哮喘患儿的肺功能及运动耐力,提升肺功能,促进运动能力的恢复及提高。

【Abstract】 Objective To explore the effect of respiratory rehabilitation training on the pulmonary function and exercise endurance in children with bronchial asthma.Methods Forty-six children patients with mild obstructive ventilation disorder after standardized treatment of bronchial asthma for more than 1 year in this hospital from September 2019 to February 2020 were selected and randomly divided into the experimental group(25 cases) and the control group(21 cases).The children in the control group received the routine treatment strictly according to the drug treatment scheme for children′s asthma recommended by GINA.The children in the experimental group received the respiratory rehabilitation training for 3 months on the basis of the medication scheme for children′s asthma recommended by the global initiative for asthma prevention and control(GINA).The pulmonary function, inspiratory muscle strength and 6-min walking test distance(6 MWD) were measured before and after intervention.Results After the intervention, the forced vital capacity(FVC) in the experimental group was 88.91%±7.74%,the one second(FEV1) was 89.96%±8.05%,the FEV1/FVC was 96.44±7.75,and which in the control group were 83.32%±8.21%,84.20%±9.46% and 87.43%±7.54% respectively.All indexes of lung function in the two groups were improved to some extent, but the experimental group was more significant and significantly higher than those in the control group(P<0.05);the inspiratory muscle strength in the experimental group was(89.06±19.22) cm H2O(1 cm H2O=0.098 kPa),which in the control group was(71.56±12.84) cm H2O,the experimental group was significantly increased compared with the control group, and the differences were statistically significant(P<0.05),the control group had no increase compared with before intervention; the 6 MWD was(571.87±53.98)m in the experimental group, the 6 MWD was(536.43±47.59)m in the control group, which in the control group was(536.43±47.59)m.the two groups were increased compared with before, but the experiment group was more significant, moreover significantly higher than that in the control group Conclusion The respiratory rehabilitation training could effectively improve the lung function and exercise endurance of children with bronchial asthma, improve the lung function, and promote the recovery and improvement of exercise ability.
