冠心病患者血清Cav-1、TRAF6水平及临床意义
Serum levels of Cav-1 and TRAF6 in patients with coronary heart disease and their clinical significance
【摘要】 目的:探讨冠心病(CHD)患者血清陷窝蛋白-1(Cav-1)、肿瘤坏死因子受体相关因子6(TRAF6)的水平及临床意义。方法:选择于我院心内科确诊的CHD患者150例为CHD组。另选同期健康体检者150例为体检对照组。ELISA检测血清Cav-1、TRAF6水平;采用二元Logistic回归分析CHD的影响因素;采用ROC分析Cav-1、TRAF6对CHD的诊断价值。结果:与体检对照组比较,CHD组糖尿病、吸烟比例、C反应蛋白(CRP)、低密度脂蛋白胆固醇(LDL-C)、Cav-1、TRAF6水平均显著升高,高密度脂蛋白胆固醇(HDL-C)水平显著降低(P<0.05或<0.01)。急性冠脉综合征(ACS)患者Cav-1、TRAF6水平均显著高于稳定型心绞痛患者(SAP)患者(P<0.05或<0.01)。随着病情严重程度的增加,CHD患者血清Cav-1、TRAF6均呈显著增加趋势(P均=0.001)。二元Logistic回归分析显示糖尿病、LDL-C、Cav-1、TRAF6是CHD的独立危险因素(OR=1.248~5.821,P<0.05或<0.01)。ROC曲线显示,Cav-1与TRAF6联合诊断CHD的价值要高于单独诊断,联合诊断曲线下面积(AUC)为0.933,敏感度87.33%,特异度93.33%。结论:血清Cav-1、TRAF6水平在CHD患者中显著升高,是CHD的独立危险因素,对CHD具有一定诊断价值。
【Abstract】 Objective:To investigate serum levels of caveolin-1(Cav-1) and tumor necrosis factor receptor-associated factor 6(TRAF6) in patients with coronary heart disease(CHD) and their clinical significance. Methods: A total of 150 CHD patients diagnosed in our department of cardiology were selected as CHD group. Another 150 healthy subjects were simultaneously enrolled as physical examination(PE) control group. Serum levels of Cav-1 and TRAF6 were detected by ELISA. Binary Logistic regression analysis was used to analyze the influencing factors of CHD. ROC was used to analyze the diagnostic value of Cav-1 and TRAF6 for CHD. Results:Compared with PE control group, there were significant rise in percentages of diabetes mellitus and smoking, levels of C reactive protein(CRP), low density lipoprotein cholesterol(LDL-C), Cav-1 and TRAF6, and significant reduction in level of high density lipoprotein cholesterol(HDL-C) in CHD group, P<0.05 or <0.01. Serum levels of Cav-1 and TRAF6 in patients with acute coronary syndrome(ACS) were significantly higher than those of patients with stable angina pectoris(SAP), P<0.05 or <0.01. Binary Logistic regression analysis indicated that diabetes mellitus, LDL-C, Cav-1 and TRAF6 were independent risk factors for CHD(OR=1.248~5.821, P<0.05 or <0.01). ROC curve indicated that diagnostic value of Cav-1 + TRAF6 for CHD was higher than that of single detection, AUC of Cav-1 + TRAF6 was 0.933, sensitivity and specificity was 87.33% and 93.33% respectively. Conclusion:Serum levels of Cav-1 and TRAF6 significantly increase in CHD patients, which are independent risk factors of CHD and possess certain diagnostic value for CHD.
