弹性固定治疗下胫腓联合损伤的临床进展
Clinical Progress of Elastic Fixation in Treatment of Lower Tibiofibular Syndesmosis Injury
【摘要】 下胫腓联合由胫腓骨远端及下胫腓四条主要韧带构成,既能维持踝关节的稳定性,又使踝关节可以产生一定的微动。踝关节损伤常伴有下胫腓联合损伤,无论是否伴有踝关节的骨折,近年来对于下胫腓联合损伤的诊治越来越受到重视。随着生物力学的发展及早期康复概念的普及,下胫腓联合螺钉固定技术治疗下胫腓联合损伤的金标准已不再适宜。弹性固定因为具有良好的生物力学特性,而受到关注及发展。目前,弹性固定的方法主要有缝合纽扣固定、韧带重建、缝合锚技术和Nice结等固定方法。弹性固定治疗下胫腓联合损伤逐渐成为主流,其更有利于下胫腓联合损伤的预后恢复,但应严格把握手术适应证及禁忌证。
【Abstract】 The lower tibiofibular syndesmosis is composed of four main ligaments of distal tibiofibular bone and lower tibiofibular bone, which can not only maintain the stability of the ankle joint, but also make the ankle joint produce a certain degree of fretting. Lower tibiofibular syndesmosis injury is often associated with ankle injury. In recent years,more and more attention has been paid to the diagnosis and treatment of lower tibiofibular syndesmosis injury, whether accompanied by ankle fracture or not. With the development of biomechanics and the popularization of early rehabilitation concept, the gold standard for the treatment of lower tibiofibular syndesmosis injury is no longer suitable. Elastic fixation has been concerned and developed because of its good biomechanical properties. At present, the main methods of elastic fixation include suture button fixation, ligament reconstruction, suture anchor technique and Nice knot. The treatment of lower tibiofibular syndesmosis injury with elastic fixation has gradually become the mainstream,which is more conducive to the prognosis and recovery of lower tibiofibular syndesmosis injury, but the surgical indications and contraindications should be strictly grasped.
