肺功能联合呼出气一氧化氮测定在儿童慢性咳嗽中的价值分析

【摘要】 目的 探讨肺功能联合呼出气一氧化氮测定在儿童慢性咳嗽中的价值分析。方法 回顾性分析2018年7月—2022年10月泰州市第四人民医院儿科治疗的132例儿童慢性咳嗽患儿的临床资料,根据病因分为3组,各44例。A组为咳嗽变异性哮喘(cough-variant asthma,CVA),B组为嗜酸性粒细胞性支气管炎(eosino‐philic bronchitis,EB),C组为上呼吸道咳嗽综合征(upper airway cough syndrome,UACS),对比3组各项参数。结果 A组呼出气一氧化氮(24.33±18.12)ppb明显高于B组、C组,差异有统计学意义(F=13.802,P<0.05);A组第一秒最大呼气容积、用力肺活量、用力呼出75%肺活量的呼气流量明显低于B组、C组,而最大呼气中期流量低于C组,差异有统计学意义(P<0.05)。3组Z5、R20、R5-R20比较,差异有统计学意义(P<0.05)。结论 肺功能和呼出气一氧化氮测定在儿童慢性咳嗽中的价值显著,能鉴别各病因之间的不同,为临床治疗提供有力依据。

【Abstract】 Objective To explore the value of lung function combined with exhaled nitric oxide measurement in children with chronic cough. Methods Retrospectively analysed of the clinical data of 132 children with chronic cough treated in the pediatric department of the Fourth People’s Hospital of Taizhou City from July 2018 to October 2022.They were divided into three groups based on the etiology, with 44 cases in each group. Group A was cough variant asthma(CVA), group B was eosinophilic bronchitis(EB), and group C was upper airway cough syndrome(UACS). Compared the parameters of the three groups. Results The exhaled nitric oxide(24.33±18.12) ppb in group A was significantly higher than that in group B and group C, the difference was statistically significant(F=13.802, P<0.05). The maximum expiratory volume, forced vital capacity, and forced expiratory flow of 75% vital capacity in the first second of group A were significantly lower than those of group B and group C, the maximum mid expiratory flow was lower than that of group C, and the difference was statistically significant(P<0.05). Compared three groups of Z5, R20, and R5-R20, the difference was statistically significant(P<0.05). Conclusion Lung function and FeNO measurements are of significant value in children with chronic cough, which can identify the differences between causes and provide a strong basis for clinical treatment.
