【摘要】 目的 探讨凝血酶原时间（prothrombin time, PT）及血小板参数[血小板分布宽度（platelet distribution width, PDW），血小板压积（platelet pressure, PCT），平均血小板体积（mean platelet volume, MPV），血小板计数（platelet count, PLT）]检验对诊断肝硬化及评估肝出血风险的价值。方法 选择2021年3月—2023年3月高邮市中医医院收治的肝硬化患者180例（肝硬化组）与健康体检者180例（健康组），均检测凝血酶原时间以及血小板参数，比较两组检测结果的差异，评估上述指标对肝硬化的诊断效能。结果 肝硬化组PT(17.86±2.25)s、PDW(21.54±1.38)%、MPV(15.20±1.95)fL均高于健康组，PCT(0.10±0.03)%、PLT(69.84±17.32)×10~9/L小于健康组，差异有统计学意义（t=32.437、45.049、23.717、21.466、33.229,P<0.05）。PT、PDW、MPV、PCT、PLT等指标在肝硬化出血风险评估中的AUC分别为0.901、1.000、0.899、0.886、0.781(P<0.05)。结论 肝硬化患者凝血酶原时间与血小板参数明显异常，该两类指标检测对肝硬化诊断效能高，且能够反映出血风险。更多还原

【Abstract】 Objective To investigate the value of prothrombin time(PT) and platelet parameters(PDW, PCT, MPV,PLT) tests in diagnosing liver cirrhosis and assessing the risk of hepatic bleeding. Methods 180 patients with liver cirrhosis admitted to Gaoyou Hospital of Traditional Chinese Medicine from March 2021 to March 2023(cirrhosis group)and 180 healthy medical checkups(healthy group) were selected. Prothrombin time and platelet parameters were detected, and the differences between the two groups were compared to assess the diagnostic efficacy of the combined application of the above indexes for liver cirrhosis. Results PT(17.86±2.25) s, PDW(21.54±1.38) %, MPV(15.20±1.95) fL in the cirrhosis group were higher than those in the healthy group, and PCT(0.10±0.03) %, PLT(69.84±17.32) ×10~9/L were smaller than those in the healthy group, the difference was statistically significant(t=32.437,45.049, 23.717, 21.466, 33.229, P<0.05). The AUC of PT、PDW、MPV、PCT、PLT in the assessment of the risk of cirrhosis with bleeding were 0.901, 1.000, 0.899, 0.886, 0.781(P<0.05). Conclusion The thromboplastin time and platelet parameters were significantly abnormal in patients with cirrhosis, and these two types of indexes are highly effective in the diagnosis of cirrhosis and can respond to the risk of bleeding.更多还原
