【摘要】 目的 探讨多囊卵巢综合征不孕女性血清25-羟基维生素D[25-(OH)D]水平与胰岛素抵抗和体外受精(IVF)妊娠结局之间的相互关系。方法 回顾性分析2018年3月至10月在兰州大学第一医院生殖医学中心就诊的多囊卵巢综合征且行IVF助孕的患者的临床资料。根据真实世界中血清25-(OH)D水平，以20ng/mL为界值将患者分为维生素D不足组和维生素D充足组。比较分析两组女性胰岛素分泌功能的相互关系，以及对妊娠结局的影响。结果 血清25-(OH)D与空腹血糖弱相关，与餐后60min血糖显著负相关，与胰岛素敏感性指数显著正相关；维生素D不足不影响IVF的胚胎质量和妊娠结局。结论 血清25-(OH)D可能主要通过影响胰岛素早期分泌而参与多囊卵巢综合征的病理生理进程。更多还原

【Abstract】 Objective To investigate the correlation of serum 25-hydroxyvitamin D [25-(OH)D] levels with insulin resistance and pregnancy outcome in in vitro fertilization(IVF) in infertile women with polycystic ovary syndrome(PCOS). Methods The clinical data of patients with PCOS who attended the Reproductive Medicine Centre of the First Hospital of Lanzhou University and underwent IVF for pregnancy were retrospectively analyzed from March to October 2018. According to the real-world serum 25-(OH)D levels, patients were divided into vitamin D deficient and vitamin D sufficient groups with a cut-off value of 20ng/mL. The interrelationship of insulin secretory function in women in the two groups was compared and analyzed, as well as the impact on pregnancy outcome. Results Serum 25-(OH)D was weakly correlated with fasting glucose, significantly negatively correlated with 60min postprandial glucose and significantly positively correlated with insulin sensitivity index; vitamin D insufficiency did not affect embryo quality and pregnancy outcome in IVF. Conclusions Serum 25-(OH)D can be involved in the pathophysiological process of PCOS mainly by influencing the early secretion of insulin.更多还原
