诱导膜结合营养指导治疗糖尿病足的疗效观察
Clinical observation of induced membrane technology combined with nutritional guidance in the treatment of diabetic foot
【摘要】 目的 讨论诱导膜技术结合营养指导治疗糖尿病足治疗方法和效果。方法 选择2019年9月-2021年9月本院105例糖尿病足患者中临床应用诱导膜技术结合营养指导治疗30例,男13例,女17例,年龄58岁-78岁,平均(67.60±1.06)岁,糖尿病病史(16.47±0.55)年,其中21例Wagner 3级,9例Wagner 4级;入院后行内科综合治疗,营养指导,血糖平稳后一期彻底清创后骨水泥植入,3周-4周后诱导膜形成后二期手术取出骨水泥,术后可吸收线缝合创面,放置引流。结果 本组30例均获得2月-2年随访,继发溃疡4例(均为Wagner 4级),其中足部截肢平面增高2例,感染控制不佳再次行诱导膜技术治疗3例;余创面愈合良好,肢体功能恢复良好。结论 诱导膜技术结合营养指导治疗糖尿病足效果良好,手术操作相对简单,创伤小,术后并发症较少,炎性指标下降明显,白蛋白指标较术前恢复。可提高糖尿病足保肢水平,提高患者生活质量,是多学科联合诊治糖尿病足的重要体现。
【Abstract】 Objective To discuss the therapeutic method and effect of induced membrane technology combined with nutritional guidance in treating withdiabetic foot. Methods From September 2019 to September 2021, Among105 cases with diabetic foot in our hospital, 30 patients, including 13 males and 17 females, aged from 58 to 78 years old, with an average age of(67.60 ± 1.06)years old and a history of diabetes mellitus(16.47 ± 0.55 years), were treated with induced membrane technology combined with nutritional guidance. Among them, 21 cases were belonging to Wagnergrade 3 and 9 cases were Wagner grade 4. After admission, patients received comprehensive medical treatment and nutritional guidance. After blood glucose was stable, bone cement was implanted after one-stage thorough debridement. After inducing membrane formation for 3-4 weeks, bone cement was removed after two-stage operation. After operation, the wound was sutured with absorbable suture and placed for drainage. Results 30 cases in this groupwere followed up for 2months to 2 years. There were 4 cases of secondary ulcer(all Wagner grade 4), including 2 cases of increased amputation level of foot. 3cases withpoor infection control were treated with induced membrane technology again. The remaining wounds healed well and the limb function recovered well. Conclusion Induced membrane technology combined with nutritional guidance for the treatment of diabetic foot is effective, with relatively simple operation, less trauma, fewer postoperative complications, inflammatory indexesdecreased significantlyand albumin recovery. It can improve the level of diabetic foot limb preservation and improve the quality of life of patients. It is an important manifestation of multidisciplinary joint diagnosis and treatment of diabetic foot.
