能谱CT成像在鉴别肺癌病理分型中的价值分析

【摘要】 目的:分析在鉴别肺癌病理分型中能谱CT成像的应用价值。资料与方法:搜集2018年5月至2021年9月经本院病理证实的54例肺癌患者的资料,取动脉期、静脉期能谱参数进行统计学分析。结果:动、静脉期3组的NICcen、dNIC、斜率λHU、 CT40keV、钙含量间差异均有统计学意义(P<0.05)。在3组两两比较中,肺鳞癌和肺小细胞肺癌NICcen、钙含量与肺腺癌间差异均有统计学意义(均P<0.05),肺鳞癌和肺小细胞肺癌能谱参数均低于肺腺癌;肺腺癌和肺鳞癌dNIC间差异有统计学意义(P<0.05)。静脉期3组的NICper间差异均有统计学意义(P<0.05);在3组两两比较中,肺鳞癌和小细胞肺癌NICcen、NICper、斜率λHU间差异有统计学意义(P<0.05)。结论:不同病理类型肺癌有不同的能谱CT参数特征,可为临床提供影像鉴别依据,为肺癌的诊治提供新的思路。

【Abstract】 Objective:To explore the application value of energy Spectrum CT in differentiating pathological types of lung cancer.Methods:The data of 54 patients with lung cancer confirmed by histopathology from May 2018 to September 2021 were collected.Energy spectrum parameters of arterial and venous phases were statistically analyzed.Results:There were statistically significant differences in NICcen, dNIC, slope λHU, CT40keV and calcium content among the three groups in arteriovenous and venous stages(P<0.05).In pairwise comparison of the 3 groups, there were statistically significant differences in NICcen and calcium content between squamous cell carcinoma and small cell lung cancer and adenocarcinoma(all P<0.05),the energy spectrum parameters of lung squamous cell carcinoma and lung small cell carcinoma were lower than those of lung adenocarcinoma; There was statistical significance in dNIC between lung adenocarcinoma and lung squamous cell carcinoma(P<0.05);There were significant differences in NICcen, NICper and slope λHU between squamous cell carcinoma and small cell lung cancer in pairwise comparison among the three groups(P<0.05).Conclusion:Different pathological types of lung cancer have different characteristics of energy spectrum CT parameters, which can provide a basis for clinical image identification and a new idea for the diagnosis and treatment of lung cancer.
