【摘要】 目的：探讨盐酸氨溴索联合纤维支气管镜吸痰治疗老年重症肺炎的效果。方法：选取2021年1月—2023年2月东营市利津县中心医院收治的80例老年重症肺炎患者作为研究对象，根据随机数字表法分为两组，各40例。对照组给予盐酸氨溴索治疗，观察组在对照组基础上联合纤维支气管镜吸痰治疗。比较两组治疗效果。结果：观察组咳嗽消失时间、体温恢复时间、住院时间短于对照组，差异有统计学意义（P<0.001）。治疗前，两组动脉血氧饱和度（SaO₂）、动脉血氧分压（PaO₂）、氧合指数（PaO₂/FiO₂）比较，差异无统计学意义（P>0.05）；治疗后，两组SaO₂、PaO₂、PaO₂/FiO₂高于干预前，且观察组高于对照组，差异有统计学意义（P<0.05）。治疗前，两组C反应蛋白（CRP）、降钙素原（PCT）、白细胞（WBC）水平比较，差异无统计学意义（P>0.05）；治疗后，两组CRP、PCT、WBC水平低于治疗前，且观察组低于对照组，差异有统计学意义（P<0.05）。观察组不良反应总发生率较对照组低，差异有统计学意义（P=0.034）。结论：酸氨溴索联合纤维支气管镜吸痰治疗老年重症肺炎的效果显著，可促进患者快速康复，改善血气指标及炎性因子水平，降低不良反应发生率。更多还原

【Abstract】 Objective:To explore the effect of ambroxol hydrochloride combined with fiberoptic bronchoscopic sputum aspiration in treatment of senile severe pneumonia.Methods:A total of 80 elderly patients with severe pneumonia who were admitted to Lijin County Central Hospital of Dongying City from January 2021 to February 2023 were selected as the study subjects.They were divided into two groups according to the random numerical table method,with 40 cases in each group.The control group was treated with ambroxol hydrochloride,and the observation group was treated with fiberoptic bronchoscopic sputum aspiration on the basis of control group.The treatment effect between the two groups was compared.Results:The cough disappearance time,body temperature recovery time and length of hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant（P<0.001）.Before treatment,there was no significant difference in arterial oxygen saturation（SaO₂）,arterial oxygen partial pressure（PaO₂） and oxygenation index（PaO₂/FiO₂） between the two groups（P>0.05）.After treatment,the SaO₂,PaO₂ and PaO₂/FiO₂ in the two groups were higher than those before intervention,and the observation group was higher than the control group,and the difference was statistically significant（P<0.05）.Before treatment,there was no significant difference in the levels of C-reactive protein（CRP）,procalcitonin（PCT） and white blood cell（WBC） between the two groups（P>0.05）.After treatment,the levels of CRP,PCT and WBC in the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant（P<0.05）.The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant（P=0.034）.Conclusion:Ambroxol hydrochloride combined with fiberoptic bronchoscopic sputum aspiration in the treatment of senile severe pneumonia has a remarkable effect,which promote the rapid rehabilitation of patients,ameliorate the blood gas indicators and inflammatory factor levels,and reduce the incidence of adverse reactions.更多还原
