显微镜下微创手术治疗高血压脑出血的效果分析
Effect Analysis of Microscopic Minimally Invasive Surgery in Treatment of Hypertensive Cerebral Hemorrhage
【摘要】 目的:探讨显微镜下微创手术治疗高血压脑出血的临床效果。方法:选取2019年1月—2021年12月黄梅县人民医院收治的60例高血压脑出血患者作为研究对象,根据患者病情及意愿分为观察组与对照组,各30例。对照组给予小骨窗开颅术,观察组给予显微镜下微创手术。比较两组临床疗效、美国国立卫生研究院卒中量表(NIHSS)评分、日常生活自理能力量表(ADL)评分、手术指标、并发症发生情况。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.001)。治疗前,两组NIHSS、ADL评分比较,差异无统计学意义(P>0.05);治疗后,观察组NIHSS评分低于对照组,ADL评分高于对照组,差异有统计学意义(P=0.001)。观察组手术时间短于对照组,术中出血量低于对照组,血肿清除率高于对照组,差异有统计学意义(P=0.001)。观察组并发症发生率低于对照组,差异有统计学意义(P=0.001)。结论:显微镜下微创手术治疗高血压脑出血有显著的效果,可改善患者的神经功能及生活质量,具有手术时间短、术中出血量少、血肿清除率高等优势,且安全性较高。
【Abstract】 Objective: To investigate the clinical effect of microscopic minimally invasive surgery in the treatment of hypertensive cerebral hemorrhage. Methods: Sixty patients with hypertensive cerebral hemorrhage admitted to Huangmei County People’s Hospital from January 2019 to December 2021 were selected as the study subjects. According to the patient’s condition and willingness, they were divided into observation group and control group, with 30 cases in each group. The control group received small bone window craniotomy. The observation group received microscopic minimally invasive surgery. Clinical efficacy, National Institute of Health Stroke Scale(NIHSS) score, Activities of Daily Living(ADL) score, surgical index and complications were compared between the two groups. Results: The total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant(P=0.001). Before treatment, there was no significant difference in NIHSS and ADL scores between the two groups(P>0.05). After treatment, NIHSS score in the observation group was lower than that in the control group, ADL score in the observation group was higher than that in the control group, and the difference was statistically significant(P=0.001). The operation time in the observation group was shorter than that in the control group, the intraoperative blood loss in the observation group was lower than that in the control group, and the hematoma clearance rate in the observation group was higher than that in the control group, the difference was statistically significant(P=0.001). The complication incidence in the observation group was lower than that in the control group, and the difference was statistically significant(P=0.001).Conclusion: Microscopic minimally invasive surgery has a significant effect on treating hypertensive cerebral hemorrhage, which can improve neurological function and life quality of patients, has the advantages of short operation time, less intraoperative blood loss, high hematoma clearance rate, and high safety.
