中国显微内镜消化系统疾病临床应用共识意见

【摘要】 随着消化道疾病早期筛查观念的普及和内镜技术的不断发展,许多高分辨率和高诊断敏感性的新型内镜技术在传统内镜的基础上应运而生。显微内镜是将消化内镜技术与显微成像相结合的一种新型内镜检查手段,以共聚焦激光显微内镜和细胞内镜为代表,可实时、无创地在细胞和亚细胞水平进行胃肠道黏膜组织学检查。近年来,显微内镜技术发展迅速,临床应用范围不断扩大,如何实现其使用过程的规范化成为亟待解决的问题。本共识在现有国内外临床实践和相关研究的基础上,以临床问题为导向、循证医学为基础,经专家组成员审阅、讨论和投票后,针对显微内镜的应用范围、诊断标准、质量控制等方面最终提出28条推荐意见,并评估其证据质量和推荐强度,以期指导显微内镜的规范化应用。

【Abstract】 With the popularization of the concept of screening of digestive diseases and the development and progression of endoscopy, many new endoscopic techniques with high resolution and high diagnostic sensitivity have emerged. Endomicroscopy is a new examination method that combines digestive endoscopy with microscopic imaging.Represented by confocal laser endomicroscopy and endocytoscopy, it can achieve non-invasive "optical biopsy" of the gastrointestinal mucosa at the cellular and subcellular levels. In recent years, with the rapid progression of endomicroscopy and the continuous expansion of its clinical use, how to standardize its application has become an urgent problem to be solved. Based on the existing clinical practice and related researches, this guideline is fully discussed by the members of the expert group and finally put forward 28 recommendations on the application range, diagnostic criteria, and quality control of endomicroscopy, and the quality of the evidence and strength of recommendations were evaluated, with a view to guiding the standardized application of endomicroscopy.
