【摘要】 嵌合抗原受体T细胞(CAR T)在血液系统恶性肿瘤的治疗上取得了显著的成功,但能否同样程度的使实体瘤患者受益仍不确定。数百项临床试验正在积极探索多种肿瘤相关抗原,但在实体瘤CAR T研究中尚未发现类似CD19特性的抗原。此外,T细胞运输效率低下、免疫抑制性肿瘤微环境、抗原识别特异性欠佳以及缺乏安全控制也是影响CAR T细胞治疗恶性肿瘤的主要障碍。本文对CAR T在血液系统恶性肿瘤和实体瘤的研究进展及毒性反应进行回顾,讨论了CAR T在恶性肿瘤治疗中的疗效、挑战以及解决方案,旨在为研究人员克服相关障碍提供参考。

【Abstract】 Chimeric antigen receptor T cells(CAR T) have achieved remarkable success in treating hematological malignancies,but whether they can benefit patients with solid tumors to the same extent remains uncertain. Hundreds of clinical trialsactively explore various tumor-associated antigens,but no antigens with CD19-like properties have been identified in the solid tumor CAR T immunotherapy. In addition,inefficient T cell transport,immunosuppressive tumor microenvironment,poor antigen recognition specificity,and lack of safety controls are also major barriers to CAR T cell therapy for malignant tumors. In this paper,we reviewed the research progress and toxicity of CAR T studies in hematological malignancies and solid tumors,and discussed the efficacy,challenges,and solutions of CAR T in treating malignancies,aiming to provide a reference for researchers to overcome the related obstacles.
