功能性电刺激联合肌筋膜疼痛触发点针刺疗法对脑卒中后痉挛型足下垂患者步态稳定性及FAC评分的影响

【摘要】 目的 研究功能性电刺激联合肌筋膜疼痛触发点针刺疗法对脑卒中后痉挛型足下垂患者步态稳定性及功能性步行量表(FAC)评分的影响。方法 前瞻性选取2019年4月至2022年10月首都医科大学附属北京潞河医院收治的116例脑卒中后痉挛型足下垂患者作为研究对象。按随机数字表法将其分为两组,每组58例。对照组采用功能性电刺激治疗,观察组采用功能性电刺激联合肌筋膜疼痛触发点针刺疗法治疗。统计分析两组治疗前后步态时空参数、FAC评分及临床痉挛指数(CSI)评分。结果 治疗后,两组步频、步幅、患侧支持相及患侧摆动相均较治疗前明显升高,且观察组步频、步幅、患侧支持相及患侧摆动相分别为(1.24±0.06)步/s、(0.45±0.04) m、(0.37±0.05) s、(0.66±0.05) s,均明显长于对照组[(1.15±0.06)步/s、(0.39±0.05) m、(0.31±0.06) s、(0.55±0.04) s],差异均有统计学意义(P<0.05)。治疗后,两组FAC评分均较治疗前明显升高,CSI评分较治疗前明显下降,且观察组的FAC评分为(4.31±0.72)分,明显高于对照组[(3.65±0.55)分],CSI评分为(6.55±1.94)分,明显低于对照组[(8.17±2.22)分],差异均有统计学意义(P<0.05)。结论 功能性电刺激联合肌筋膜疼痛触发点针刺疗法能够有效提高脑卒中后痉挛型足下垂患者步态稳定性及肌张力,有利于患者步行功能恢复,值得临床推广应用。

【Abstract】 Objective To investigate the effects of functional electrical stimulation combined with myofascial pain trigger acupuncture on gait stability and functional walking scale(FAC) score in patients with spastic foot drop after stroke. Methods A total of 116 patients with spastic foot drop after stroke admitted to Beijing Luhe Hospital, Capital Medical University from April 2019 to October 2022 were selected as the research objects and divide them into two groups according to the random number table method, with 58 cases in each group. The control group was treated with functional electrical stimulation, and the observation group was treated with functional electrical stimulation combined with myofascial pain trigger point acupuncture. The temporal and spatial parameters of gait, FAC score and clinical spasm index(CSI) score before and after treatment were statistically analyzed. Results After treatment, the stride frequency, stride length, the affected side support phase and the affected side swing phase of the two groups were significantly increased compared to before treatment, and those in the observation group were(1.24±0.06) steps/s,(0.45±0.04) m,(0.37±0.05) s,(0.66±0.05) s, respectively, which were significantly longer than those in the control group[(1.15±0.06) steps/s,(0.39±0.05) m,(0.31±0.06) s,(0.55±0.04) s], the differences were statistically significant(P<0.05). FAC score was significantly increased compared to before treatmentin both groups after treatment, while CSI score was significantly decreased compared to before treatment, the FAC score of the observation group was(4.31±0.72) points, which was significantly higher than that in the control group [(3.65±0.55) points], while the CSI score was(6.55±1.94) points, which was significantly lower than that in the control group [(8.17±2.22) points], and the differences were statistically significant(P<0.05). Conclusion Functional electrical stimulation combined with myofascial pain trigger point acupuncture therapy can effectively improve the gait stability and muscle tension of patients with spastic foot drop after stroke, which is conducive to the recovery of walking function of patients, and is worthy of clinical application.
