【摘要】 目的 探讨股骨头坏死（ONFH）塌陷后双侧髋关节坐骨股骨间隙（IFS）、股方肌间隙（QFS）的变化情况及坐骨股骨撞击（IFI）的发病情况，分析ONFH患者并发IFI的风险。方法 在轴位T₁WI序列上分别测量127例ONFH患者（病例组，包括未塌陷和塌陷侧）及74例双髋关节MRI扫描正常的受检者（对照组）髋部IFS及QFS并判断是否伴发IFI及半脱位。结果 未塌陷组的IFS、QFS均小于塌陷组及对照组；病例组中女性IFI发病率高于男性；女性塌陷组IFI发病率高于未塌陷组；IFI者出现半脱位频率高于无IFI者。结论 ONFH一侧塌陷后，未塌陷侧较塌陷侧IFS、QFS减小；ONFH中女性较男性更易发生IFI;女性ONFH患者塌陷组较未塌陷组更易罹患IFI;IFI者出现半脱位频率较无IFI者高，髋关节动态不稳定可能与ONFH患者IFI发病相关。更多还原

【Abstract】 Objective To investigate the changes of ischiofemoral space（IFS） and quadratus femoris space（QFS） and the incidence of ischiofemoral impingement（IFI） after the collapse of osteonecrosis of the femoral head（ONFH）,and to explore the risk of IFI in patients with ONFH. Methods The hip IFS and QFS were measured on axial T₁WI sequence in 127 patients with ONFH（case group, including uncollapsed and collapsed sides）and 74 subjects with normal MRI scan of bilateral hip joints（control group）and the presence of IFI and subluxation was determined.Results The IFS and QFS of the uncollapsed group were lower than those of the collapsed group and the control group; the incidence of IFI in female was higher than that in male; the incidence of IFI in female collapsed group was higher than that in uncollapsed group; the frequency of subluxation in patients with IFI was higher than that in patients without IFI. Conclusion After the collapse of one side of ONFH,the IFS and QFS of the uncollapsed side are lower than those of the collapsed side; the female was more likely to develop IFI than the male in ONFH;IFI is more likely to occur in the collapsed group than in the uncollapsed group in female ONFH patients; the frequency of subluxation in patients with IFI is higher than that in patients without IFI.The dynamic instability of hip joint may be related to the pathogenesis of IFI in patients with ONFH.更多还原
