【摘要】 目的 探讨非酒精性脂肪肝(NAFLD)与冠状动脉周围脂肪组织(PCAT)高衰减之间的关系。方法 选取行冠状动脉CT血管成像(CCTA)的209例患者。根据肝/脾CT比值是否≤1.0分为NAFLD组及非NAFLD组。比较2组一般资料、斑块性质、易损斑块特征、PCAT衰减值。采用二元logistic回归分析NAFLD与PCAT高衰减之间的相关性。结果 NAFLD患者糖尿病及高脂血症患病率、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、谷氨酰转肽酶(GGT)和尿酸水平均高于非NAFLD患者(P<0.05);NAFLD患者非钙化斑块、正性重构、低密度斑块及点状钙化发生率均高于非NAFLD患者(P<0.05)。调整传统心血管危险因素、正性重构及阻塞性斑块后,NAFLD仍与左前降支(LAD)-PCAT高衰减独立相关。结论 NAFLD与LAD-PCAT高衰减相关,独立于传统心血管危险因素。更多还原

【Abstract】 Objective To investigate the relationship between non-alcoholic fatty liver disease(NAFLD) and pericoronary adipose tissue(PCAT) high attenuation.Methods A total of 209 patients who underwent coronary computed tomography angiography(CCTA) were enrolled.The patients were divided into NAFLD group and non-NAFLD group according to the liver/spleen CT ratio≤1.0.The general data,plaque properties,vulnerable plaque characteristics,and PC AT attenuation value were compared between the two groups.Binary logical regression was used to analyze the correlation between NAFLD and PCAT high attenuation.Results The prevalence of diabetes and hyperlipidemia,the levels of aspartate aminotransferase(AST),alanine transaminase(ALT),glutamyl transpeptidase(GGT) and uric acid in NAFLD patients were higher than those in non-NAFLD patients(P<0.05);The incidence of non-calcified plaques,positive remodeling,low attenuation plaque,and spotty calcification in patients with NAFLD were higher than those in non-NAFLD patients(P <0.05).After adjusting the traditional cardiovascular risk factors,positive remodeling and obstructive plaque,NAFLD was still independently associated with the left anterior descending(LAD)-PCAT high attenuation.Conclusion NAFLD is associated with high attenuation of LAD-PCAT independent of traditional cardiovascular risk factors.更多还原
