健侧大脑半球动脉自旋标记灌注定量与急性脑梗死第90天预后的相关性研究
Correlation between contralateral hemispheric cerebral blood flow measured by arterial spin labeling and outcome at day 90 in patients with acute ischemic stroke
【摘要】 目的 研究急性脑梗死患者较高的健侧大脑半球脑血流量(cCBF)是否与其第90天良好预后相关。方法 选取诊断为前循环急性缺血性脑卒中并行MRI检查患者113例,分别通过动脉自旋标记(ASL)技术及扩散加权成像(DWI)获得cCBF及病灶DWI大小,并评估首日美国国立卫生研究院卒中量表(NIHSS)、次日NIHSS、第5天NIHSS、第90天卒中前改良Rankin量表(mRS),根据中位cCBF分为高cCBF组和低cCBF组。结果采用首日NIHSS、次日NIHSS、第5天NIHSS、第90天mRS进行评估,经多变量logistic回归检验较高的cCBF是否与第90天的神经功能良好预后相关。结果 113例患者发病至MRI检查时间5.1(4~7.6) h;首日NIHSS评分14(10~17)。中位cCBF为29.3(25.7~48.8) mL/(100 g·min)。较高的cCBF预测第90天的良好预后[比值比3.425,95%置信区间(CI)1.639~7.157,P<0.001]。结论 急性脑梗死患者较高的cCBF与其第90天良好预后相关。
【Abstract】 Objective To investigate the correlation between higher contralateral cerebral blood flow(cCBF) and the 90 day good outcome in patients with acute ischemic stroke.Methods A total of 113 patients diagnosed with anterior circulation acute ischemic stroke and underwent MRI scans were included.The lesion cCBF via arterial spin labeling(ASL) and volume via diffusion weighted imaging(DWI),National Institutes of Health Stroke Scale(NIHSS) on the first day,NIHSS on the next day,NIHSS on the fifth day,and the day 90 modified Rankin scale(mRS) were further obtained.According to the median cCBF,all patients were divided into high cCBF group and low cCBF group.All NIHSS scores on the first,second,and fifth day and mRS at 90-day were used to evaluate and predict the relationship between high cCBF and outcome at day 90 via multivariate logistic regression.Results Of 113 patients,the time from onset to MRI scanning was 5.1(4-7.6) hours.The NIHSS score on the first day was 14(10-17).The median cCBF was 29.3(25.7-48.8) mL/(100 g·min).Higher cCBF predicted a good outcome at day 90 [odds ratio was 3.425,95% confidence interval(CI) 1.639-7.157,P<0.001].Conclusion Higher cCBF is correlated with the day 90 good outcome in patients with acute ischemic stroke.
