“三低”在冠状动脉与头颈部血管“一站式”CT扫描的价值

【摘要】 目的 探讨小体质量指数(BMI)(≤23 kg/m2)患者在Revolution 16 cm宽体探测器CT低电压、低流速、低对比剂用量行冠状动脉与头颈部CT血管成像(CTA)“一站式”扫描的临床应用价值。方法 回顾性分析100例行冠状动脉和头颈部CTA检查患者,将其分为A、B 2组。对2组图像进行主、客观评价,并比较2组图像的主观评分、冠状动脉3支主干和头颈部血管主干的CT值、对比噪声比(CNR)、背景噪声和辐射剂量。结果 A、B 2组冠状动脉CTA和头颈部CTA图像的主观评分、CT值和CNR均无统计学差异(P>0.05)。2组患者的容积CT剂量指数(CTDIvol)和有效剂量(ED)均有统计学差异(P<0.05)。结论 Revolution16 cm宽体探测器CT采用80 kV低电压、结合低流速及低对比剂用量在小BMI患者中行冠状动脉和头颈部血管“一站式”CT扫描,能保证与常规扫描图像质量无明显差异,同时能明显降低对比剂用量及辐射剂量,降低对比剂外渗风险。

【Abstract】 Objective To explore the clinical value of "one-stop" scanning of coronary artery and head and neck computed tomography angiography(CTA) for patients with small body mass index(BMI)(≤23 kg/m2) in Revolution 16 cm wide-body detector CT with low voltage,low flow rate,and low contrast medium dosage.Methods A retrospective analysis of 100 patients who underwent coronary artery and head and neck CTA examinations was performed,and they were divided into group A and group B.Subjective and objective evaluation was conducted on the two groups of images.Then the subjective scores,the CT values of the three main coronary arteries and the main blood vessels of the head and neck,and the contrast-to-noise ratio(CNR),background noise and radiation dose of the two groups of images were compared.Results The comparison found that the subjective score,CT value and CNR of coronary artery CTA and head and neck CTA images of the A and B groups were not statistical difference(P>0.05).The volume CT dose index(CTDIvol) and effective dose(ED) were significant statistical difference between the two groups(P <0.05).Conclusion Use the Revolution 16 cm wide-body detector CT with 80 kV low voltage,low flow rate and low contrast medium dosage to perform "one-stop" CT scanning of coronary artery and blood vessels of the head and neck in patients with small BMI,which can ensure that the image quality is not obvious from conventional scans.At the same time,it can significantly reduce the amount of contrast medium and radiation dose,and reduce the risk of contrast medium extravasation.
