中青年人腰椎骨密度与椎旁肌及肝脏脂肪含量相关性研究

【摘要】 目的 探讨中青年人腰椎骨密度(BMD)、椎旁肌脂肪含量及肝脏脂肪含量的相关性。方法 选取196例行腹部CT检查的中青年人,应用定量CT(QCT)测量腰椎BMD、肝脏脂肪含量和椎旁肌(多裂肌、竖脊肌、腰大肌)脂肪含量。按肝脏脂肪含量分4组,比较不同组间腰椎BMD和椎旁肌脂肪含量的差异。按腰椎BMD分3组(正常组、低骨量组和骨质疏松组),比较不同组间肝脏和椎旁肌脂肪含量的差异。用Pearson或Spearman相关性分析和多元逐步回归分析评估腰椎BMD、肝脏及椎旁肌脂肪含量的关系。结果 肝脏脂肪含量越高,腰椎BMD越低,竖脊肌和多裂肌脂肪含量越高,腰大肌脂肪含量差异无统计学意义。腰椎BMD与肝脏脂肪含量(r=-0.208,P<0.05)、腰大肌、多裂肌、竖脊肌脂肪含量(r=-0.307、-0.353、-0.329,P<0.05)呈负相关。年龄是腰椎BMD的独立影响因素(R~2=0.461,F=168.013,P<0.001)。结论 中青年人腰椎BMD与肝脏脂肪含量及椎旁肌脂肪含量呈负相关。

【Abstract】 Objective To investigate the correlation among bone mineral density(BMD) of lumbar vertebrae,fat content of paravertebral muscle,and liver fat content in middle-aged and young people.Methods A total of 196 middle-aged and young people who underwent abdominal CT examination were selected.The BMD of lumbar vertebrae,fat content of liver and paravertebral muscles(multifidus muscle,erector spinae muscle and psoas muscle) were measured by quantitative CT(QCT).People were divided into 4 groups according to liver fat content,the differences of lumbar vertebrae BMD and paravertebral muscles fat content were compared.After that,people were divided into 3 groups(normal bone mass group,bone mass reduction group and osteoporosis group) according to lumbar vertebrae BMD,the differences of liver and paravertebral muscles fat content were compared.The relationship among BMD of lumbar vertebrae,fat content of liver and paravertebral muscles were-assessed with Pearson or Spearman correlation analysis and multiple stepwise regression analysis.Results People with higher liver fat content had lower lumbar vertebrae BMD,higher fat content of erector spinae muscles and multifidus muscles.There was no significant difference in fat content of psoas muscles.The BMD of lumbar vertebrae was negatively correlated with liver fat content(r=-0.208,P<0.05),psoas muscles,multifidus muscles and erector spinae muscles(r=-0.307.-0.353,-0.329,P<0.05).Age was an independent impact factor of lumbar vertebrae BMD(R~2=0.461,F=168.013,P <0.001).Conclusion BMD of lumbar vertebrae is negatively correlated with liver fat content and paravertebral muscles fat content in middle-aged and young people.
