地氟烷和七氟烷对术后认知功能障碍影响的荟萃分析

【摘要】 目的:评价地氟烷和七氟烷对术后认知功能障碍的影响。方法:计算机检索PubMed、Embase、Cochrane Library、中国知网、万方数据库和维普中文期刊数据库,收集术中采用地氟烷和七氟烷维持全身麻醉对患者术后认知功能障碍影响的随机对照试验研究,检索年限为建库至2023年5月。根据纳入及排除标准筛选文献、提取资料并评价文献质量,采用RevMan 5.3和Stata 15.1软件对纳入文献进行分析。结果:共纳入13篇文献,包括1 046例患者,其中术中采用地氟烷523例,采用七氟烷523例。荟萃分析结果显示,地氟烷组术后1 h (SMD=1.65,95%CI:0.70~2.60,P<0.001)、术后3 h(SMD=0.45,95%CI:0.13~0.76,P=0.006)、术后6 h (SMD=1.15,95%CI:0.18~2.12,P=0.02)、术后12 h (SMD=0.94,95%CI:0.18~1.70,P=0.01)、术后24 h (SMD=0.20,95%CI:0.06~0.33,P=0.005)的简易精神状态检查评分均高于七氟烷组,地氟烷组的睁眼时间(SMD=-2.95,95%CI:-4.42~-1.47,P<0.001)短于七氟烷组。结论:与七氟烷相比,地氟烷能更有效地减少术后认知功能障碍的发生,并且苏醒时间短。

【Abstract】 Objective: To evaluate the effects of desflurane and sevoflurane on postoperative cognitive dysfunction.Methods: Randomized controlled trial studies on the effects of intraoperative maintenance of general anesthesia with desflurane and sevoflurane on patients’ postoperative cognitive dysfunction were collected through a computerized search of PubMed, Embase, Cochrane Library, China National Knowledge Infrastructure, Wanfang Database and Wipro Chinese Journal Database from the year of the library’s establishment to May 2023. Papers were screened,extracted and evaluated for quality according to the inclusion and exclusion criteria, and the included papers were analyzed using RevMan 5.3 and Stata 15.1 software. Results: A total of 13 papers were included, including 1,046patients, of which 523 were treated with intraoperative desflurane and 523 with sevoflurane. The results of meta-analysis showed that the mini-mental state examination scores of the desflurane group were higher than those of the sevoflurane group at 1 hour after surgery(SMD=1.65, 95% CI: 0.70-2.60, P<0.001), 3 hours after surgery(SMD=0.45, 95% CI: 0.13-0.76, P=0.006), 6 hours after surgery(SMD=1.15, 95% CI: 0.18-2.12, P=0.02), 12 hours after surgery(SMD=0.94, 95% CI: 0.18-1.70, P=0.01), and 24 hours after surgery(SMD=0.20,95% CI: 0.06-0.33, P=0.005). The time to open the eyes in the desflurane group(SMD=-2.95, 95% CI:-4.42--1.47,P<0.001) was shorter than in the sevoflurane group. Conclusion: Desflurane is more effective in reducing postoperative cognitive dysfunction and has a shorter awakening time than sevoflurane.
