脑卒中患者下肢功能障碍康复的相关因素研究
Related factors of rehabilitation of lower extremity dysfunction in stroke patients
【摘要】 目的:探讨脑卒中患者下肢功能障碍康复的相关因素。方法:选取2021年11月—2022年6月在广州中医药大学第三附属医院康复科住院的80例脑卒中患者作为研究对象,根据患者下肢功能状态分为预后较差组(Brunnstrom分期为Ⅰ~Ⅲ期)和预后较好组(Brunnstrom分期为Ⅳ~Ⅵ期)。预后较差组41例,其中男性27例,女性14例,平均年龄(71±12)岁;预后较好组39例,其中男性26例,女性13例,平均年龄(70±11)岁。收集研究对象的一般资料、生活饮食习惯、临床资料、既往史和心理状态等资料。采用多因素Logistic回归模型分析可能影响脑卒中患者下肢功能障碍康复的相关因素。结果:多因素Logistic回归分析显示,饮酒(OR=0.09,95%CI:0.02~0.56)、运动量(OR=19.47,95%CI:2.45~154.68)、肌张力(OR=20.58,95%CI:2.00~211.45)、疼痛(OR=0.07,95%CI:0.13~0.39)、抑郁(OR=0.20,95%CI:0.05~0.85)、焦虑(OR=0.14,95%CI:0.02~0.83)均为可能影响脑卒中后下肢功能障碍康复的相关因素(均P<0.05),其中运动量多的卒中患者出现预后较好的机会是运动量少的患者的19.47倍,肌张力正常的卒中患者出现预后较好的机会是肌张力下降患者的20.58倍。结论:脑卒中患者下肢功能障碍康复的相关因素较多,运动量的提高和肌张力的正常化对下肢功能障碍的康复有利。
【Abstract】 Objective: To investigate the factors related to the rehabilitation of lower extremity dysfunction in stroke patients. Methods: From November 2021 to June 2022, 80 patients with stroke who were hospitalized in the Rehabilitation Department of the Third Affiliated Hospital of Guangzhou University of Chinese Medicine were selected as the research subjects, and the patients were divided into a poor prognosis group(Brunnstrom stageⅠ-Ⅲ) and a better prognosis group(Brunnstrom stage Ⅳ-Ⅵ) according to their functional status of the lower extremity. There were 41 cases in the poor prognosis group, including 27 males and 14 females, with an average age of(71±12) years; 39 cases in the better prognosis group, including 26 males and 13 females, with an average age of(70 ± 11) years. The general information, living and eating habits, clinical data, past history and psychological state of the research subjects were collected. Multivariate logistic regression model was used to analyze the related factors that may affect the rehabilitation of lower extremity dysfunction in stroke patients.Results: Multivariate logistic regression analysis showed that alcohol consumption(OR=0.09, 95%CI: 0.02-0.56),exercise(OR=19.47, 95%CI: 2.45-154.68), muscle tone(OR=20.58, 95%CI: 2.00-211.45), pain(OR=0.07, 95%CI: 0.13-0.39), depression(OR=0.20, 95%CI: 0.05-0.85), anxiety(OR=0.14, 95%CI: 0.02-0.83) were all related factors that may affect the rehabilitation of lower extremity dysfunction after stroke(all P<0.05). Stroke patients with more exercise were 19.47 times more likely to have a better prognosis than those with less exercise, and stroke patients with normal muscle tone were 20.58 times more likely to have a better prognosis than those with decreased muscle tone. Conclusion: There are many factors related to the rehabilitation of lower extremity dysfunction in stroke patients, and the improvement of exercise amount and the normalization of muscle tone are beneficial to the rehabilitation of lower extremity dysfunction.
