新生儿消化道手术后喂养不耐受的危险因素
Risk factors for feeding intolerance after neonatal gastrointestinal surgery
【摘要】 目的:探讨新生儿消化道手术后喂养不耐受的危险因素。方法:回顾性分析2020年5月—2022年5月汕头大学医学院第二附属医院78例行消化道手术的新生儿的临床资料。按照喂养不耐受诊断标准分为喂养不耐受组(44例)和喂养耐受组(34例),分析患儿喂养不耐受的临床表现及围手术期的监测指标。采用多因素Logistic回归分析筛选新生儿消化道手术后喂养不耐受的危险因素。结果:78例消化道手术后新生儿中,喂养不耐受的发生率为56.4%(44/78)。多因素Logistic回归分析表明,血清白蛋白低(OR=0.511,95%CI:0.284~0.920,P=0.025)及血红蛋白低(OR=0.839,95%CI:0.732~0.962,P=0.012)为新生儿消化道手术后喂养不耐受的危险因素。结论:新生儿消化道手术后喂养不耐受的发生率较高,低蛋白血症和贫血是新生儿消化道手术后喂养不耐受的危险因素。
【Abstract】 Objective: To investigate the risk factors of feeding intolerance after gastrointestinal surgery in neonates. Methods: The clinical data of 78 neonates who underwent gastrointestinal surgery in the Second Affiliated Hospital of Shantou University Medical College from May 2020 to May 2022 were retrospectively analyzed. The infants were divided into feeding intolerance group(44 cases) and feeding tolerance group(34cases) according to the diagnostic criteria of feeding intolerance, and the clinical manifestations of feeding intolerance and perioperative monitoring indexes of the infants were analyzed. Multi-factor logistic regression analysis was used to screen the risk factors for feeding intolerance after gastrointestinal surgery in neonates.Results: The incidence of feeding intolerance was 56.4%(44/78) among 78 neonates after gastrointestinal surgery.Multifactorial logistic regression analysis showed that low serum albumin(OR=0.511, 95% CI: 0.284-0.920, P=0.025) and low hemoglobin(OR=0.839, 95% CI: 0.732-0.962, P=0.012) were risk factors for feeding intolerance after gastrointestinal surgery in neonates. Conclusion: The incidence of feeding intolerance after neonatal gastrointestinal surgery is high, and hypoproteinemia and anemia are risk factors for feeding intolerance after neonatal gastrointestinal surgery.
