【摘要】 目的 探讨下乳涌泉汤与手指点穴联合治疗产后缺乳的临床效果。方法 选取2020年1月—2022年1月期间河南省周口市中医院妇产科收治的产后缺乳患者90例作为研究对象，按随机数字表法分为对照组和研究组，每组各45例。对照组给予下乳涌泉汤治疗，研究组在对照组基础上加用手指点穴治疗。连续治疗14 d后，观察比较两组患者临床疗效、不良反应发生率，治疗前后中医证候积分及血雌二醇（Estradiol, E₂）、泌乳素（Prolactin, PRL）的变化。结果 治疗后研究组临床总有效率95.56%（43/45）明显高于对照组80.00%（36/45）,差异有统计学意义（P<0.05）。治疗后两组患者烦躁易怒、胸胁胀闷、身有微热、食欲不振各项积分均较治疗前降低，差异有统计学意义（P<0.05）;且研究组低于对照组，差异有统计学意义（P<0.05）。治疗后两组患者E₂水平较治疗前降低，PRL水平较治疗前升高，差异有统计学意义（P<0.05）;且研究组E₂明显低于对照组、PRL水平均明显高于对照组，差异有统计学意义（P<0.05）。治疗期间，研究组不良反应发生率13.33%（6/45）与对照组8.89%（4/45）比较，差异无统计学意义（P>0.05）。结论 下乳涌泉汤与手指点穴联合治疗产后缺乳的效果显著，改善产妇肝郁气滞的症候，调节PRL、E₂水平，且具有良好的安全性。更多还原

【Abstract】 Objective To observe the clinical effect of Xiaru Yongquan Decoction combined with finger pressing of acupoints on postpartum hypogalactia.Methods Ninety patients with postpartum hypogalactia treated in the Department of Obstetrics, Zhoukou Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were selected and assigned into an observation group（n=45）and a control group（n=45）according to the random number table method.The control group was treated with Xiaru Yongquan Decoction, and the observation group was treated with Xiaru Yongquan Decoction combined with finger pressing of acupoints.After treatment for 14 days, the clinical efficacy and incidence of adverse reactions were compared between the two groups.The TCM syndrome score and serum levels of estradiol（E₂）and prolactin（PRL）were determined before and after treatment.Results The total response rate in the observation group was 95.56%（43/45）,which was higher than that（80.00%,36/45）in the control group（P<0.05）.After treatment, the scores of irritability, chest tightness, slight fever, and loss of appetite in the two groups were lower than those before treatment（P<0.05）,and the observation group had lower scores than the control group（P<0.05）.The treatment in both groups lowered the level of E₂ and elevated the level of PRL（P<0.05）,and the observation group had lower E₂ level and higher PRL level than the control group after treatment（P<0.05）.The incidence of adverse reactions in the observation group was 13.33%（6/45）,with no statistical significance compared with that（8.89%,4/45）in the control group（P>0.05）.Conclusion Xiaru Yongquan Decoction combined with finger pressing of acupoints demonstrates a significant therapeutic effect on postpartum hypogalactia.This therapy can mitigate the syndrome of liver depression and qi stagnation and regulate PRL and E₂ levels, with good safety.更多还原
