肿瘤专科医院护士的患者安全胜任力现状及影响因素分析
Current Situation and Influencing Factors of Nurses’ Safety Competency in Cancer Hospitals
【摘要】 目的:调查肿瘤专科医院护士的患者安全胜任力现况,并进一步分析其影响因素。方法:采用按比例分层整群抽样法,选取北京、河北、山西、青海、福建等14家三级甲等肿瘤专科医院护士为研究对象。应用一般资料问卷和患者安全胜任力护理人员自评量表进行横断面调查。比较护士的患者安全胜任力总分及态度、系统、知识、技能因素各维度得分。采用多因素Logistic回归模型分析护士的学历、职称、职务、是否接受过安全教育、每周工作时长对患者安全胜任力的影响。结果:肿瘤专科医院护士的患者安全胜任力得分中位数为135.00(120.00,143.00)分,条目均分中位数为4.66(4.14,4.93)分;各维度均分由低到高依次为:态度因素、系统因素、知识因素、技能因素;不同学历、职称、职务、是否接受过安全教育、每周工作时长的护士患者安全胜任力得分之间的差异均具有统计学意义(P <0.05);多因素Logistic回归分析结果显示学历(本科:OR=1.298,95%CI:1.018~1.655,P=0.035;研究生及以上:OR=14.575,95%CI:4.361~48.709,P <0.001),是否参加患者安全教育(OR=1.537,95%CI:1.139~2.074,P=0.005),每周工作时长(35~40 h:OR=0.521,95%CI:0.382~0.711,P <0.001;> 40 h:OR=0.413,95%CI:0.299~0.570,P <0.001)是影响患者安全胜任力的因素。结论:肿瘤专科医院护士患者安全胜任力处于中等水平,护士患者安全胜任力影响因素有每周工作时长、学历、职务及是否参加患者安全教育,在临床护理安全胜任力培训中,应针对不同人群差异给予针对性培训。
【Abstract】 Objective: To investigate the current situation of patient safety competency(PSC) of nurses in cancer hospitals, and further analyze its influencing factors. Methods: Using the proportional stratified cluster sampling method, we selected nurses from 14 level A tertiary cancer hospitals in Beijing, Hebei, Shanxi, Qinghai and Fujian as the study subjects. A cross-sectional survey was conducted using a general data questionnaire and the PSC Self-Rating Scale of Nurses. The total PSC scores of nurses and the scores of dimensions(attitude, system, knowledge and skill) were compared among nurses with different characteristics. A multivariate Logistic regression model was used to analyze the impacts of education, professional title, position, whether they have received safety education, and weekly working hours on PSC of nurses. Results: The median PSC score of cancer nurses was 135.00(120.00, 143.00), the median score of each item was 4.66(4.14, 4.93). Dimensions with their average PSC scores in an ascending order were attitude, system, knowledge and skill. There was statistically significant difference in the PSC score among nurses with different education, professional titles, positions, weekly working hours, and among nurses with or without safety education(P < 0.05). The results of multivariate Logistic regression analysis showed that education background(bachelor’s degree: OR = 1.298, 95% CI = 1.018 ~ 1.655, P = 0.035; master’s degree or higher: OR = 14.575, 95% CI = 4.361 ~ 48.709, P < 0.001), access to safety education(OR = 1.537, 95% CI = 1.139 ~ 2.074, P =0.005), and weekly working hours(35 ~ 40 h: OR = 0.521, 95% CI = 0.382 ~ 0.711, P < 0.001; > 40 h: OR = 0.413, 95% CI = 0.299~ 0.570, P < 0.001) were the factors affecting PSC. Conclusion: The PSC of nurses in cancer hospitals is at an intermediate level. The influencing factors of PSC of nurses include weekly working hours, education, position, and access to safety education. In the PSC training, targeted training should be given to nurses with different characteristics.
