肺部超声用于肺结核诊断的临床研究
Clinical study of pulmonary ultrasound in diagnosis of pulmonary tuberculosis
【摘要】 目的 探讨肺部超声用于肺结核诊断的临床研究。方法 将2022年7月至2023年7月在廊坊市第三人民医院及合作医院治疗的76例疑似肺结核患者作为研究对象,均实施肺部超声检查及CT检查,以支气管内镜病理活检结果为金标准,对比肺部超声、CT对于肺结核的诊断效能、各项检查指标、超声征象。结果 根据金标准,肺结核47例,细菌性肺炎31例。肺部超声对肺结核诊断的灵敏度为91.49%,特异度90.32%;CT对肺结核诊断的灵敏度为95.74%,特异度96.77%。肺部超声等待时间、检查时间、出具诊断结果时间均短于CT(P<0.05);肺结核和细菌性肺炎患者气支气管征、碎片征、瀑布征、血流信号比例比较,差异有统计学意义(P<0.05)。结论 肺部超声用于肺结核诊断的临床价值较高,对肺结核的诊断效能较高,能有效鉴别肺结核征象,且操作简便、快捷、结果迅速,值得优先选择。
【Abstract】 Objective:To investigate the clinical study of pulmonary ultrasound in the diagnosis of pulmonary tuberculosis. Methods:76 patients with suspected pulmonary tuberculosis treated in the Third People’s Hospital of Langfang and partner hospitals from July 2022 to July 2023 were selected as the study objects. All of them were examined by pulmonary ultrasound and CT, and the diagnostic efficacy, examination indexes and ultrasonic signs of pulmonary ultrasound and CT for pulmonary tuberculosis were compared with the results of bronchoendoscopic pathological biopsy as the gold standard. Results:According to the gold standard, there were 47 cases of pulmonary tuberculosis and 31 cases of bacterial pneumonia. The sensitivity and specificity of pulmonary ultrasound for pulmonary tuberculosis diagnosis were 91.49% and 90.32%. The sensitivity and specificity of CT in the diagnosis of pulmonary tuberculosis were 95.74% and 96.77%. The waiting time, examination time and diagnosis result issuance time of pulmonary ultrasound were shorter than those of CT(P<0.05). There were significant differences in the proportion of pneumobronchial sign, fragment sign, waterfall sign and blood flow signal between pulmonary tuberculosis and bacterial pneumonia(P<0.05). Conclusion:Pulmonary ultrasound is of high clinical value in the diagnosis of pulmonary tuberculosis, has high diagnostic efficiency, can effectively identify the signs of pulmonary tuberculosis, and is simple, quick and rapid, worthy of priority choice.
