基于关联规则和聚类分析探讨针灸治疗神经根型颈椎病的选穴规律
Exploration of the Selection Rules of Acupoint for the Treatment of Cervical Spondylotic Radiculopathy Based on the Association Rules and Cluster Analysis
【摘要】 【目的】通过关联规则和聚类分析等数据挖掘技术对相关随机对照试验的分析,探究针灸治疗神经根型颈椎病的临床腧穴应用规律。【方法】检索2012年1月起至2022年8月间中国知网期刊全文数据库(CNKI)、万方学术期刊全文数据库(Wanfang)、维普中文科技期刊数据库(VIP)、中国生物医学数据库(CBM)、美国生物医学信息检索系统(PubMed)、荷兰医学文摘(Embase)、国际循证医学图书馆(Cochrane Library)、科学开放资源库(Sci-hub)等各大数据库中收录的关于针灸治疗神经根型颈椎病的随机对照临床研究,建立资料数据库,对数据进行频次、关联规则和聚类分析,并构建可视化网络图。【结果】本研究共纳入106首针灸处方,累积样本量6 369例,腧穴总频数为772次,应用频次最高腧穴为风池(80次);干预方法以针刺与推拿结合治疗最常见,选穴部位以头面颈肩部和四肢部的分布为多;以交会穴的应用最广;常用经脉以大肠经、胆经、小肠经和督脉为主。【结论】通过数据挖掘对腧穴配伍、部位、特定穴、经脉应用规律及干预方法的探究,可为神经根型颈椎病的临床治疗提供新的思路。
【Abstract】 Objective To explore the clinical application rules of acupuncture for the treatment of cervical spondylotic radiculopathy(CSR) through the analysis of relevant randomized controlled trials by data mining techniques including association rules and cluster analysis. Methods The randomized controlled clinical studies on acupuncture and moxibustion for CSR were searched from January 2012 to August 2022 in major databases such as CNKI,Wanfang,VIP,CBM,PubMed,Embase,Cochrane Library and Sci-hub. A information database was established, the frequency, association rules and clustering of the data were analyzed, and a visual network diagram was constructed. Results A total of 106 acupuncture prescriptions were included in this study, with a cumulative sample size of 6 369 cases and a total acupoint frequency of 772. The most frequently applied acupoint was Fengchi(GB20,used for 80 times);the intervention method was most commonly combined with acupuncture and tuina therapy, with the distribution of selected acupoints on the head, face, neck, shoulder and extremities;the confluence points were the most widely used;the commonly used meridians were mainly the large intestine meridian,the bile meridian,the small intestine meridian and the governor vessel(GV1). Conclusion This study explores the application rules of acupoint combinations, sites, specific points, channels and intervention methods through data mining,which may provide new ideas for the clinical treatment of CSR
