1990年和2019年中国归因于肾功能不全的心血管疾病负担分析

【摘要】 目的 评估1990年和2019年中国归因于肾功能不全的心血管疾病(CVD)负担,为CVD的防治提供理论依据。方法 基于全球疾病负担(GBD)2019数据库,采用Excel软件分析我国不同年龄、性别人群归因于肾功能不全的CVD死亡率、过早死亡损失寿命年(YLL)和伤残调整寿命年(DALY)。结果 2019年,我国归因于肾功能不全的CVD死亡人数和DALYs分别为37.7万例(95%UI:28.5~48.0)和747.6万人·年(95%UI:579.7~939.1),分别较1990年增加了151.3%和103.8%;年龄标化死亡率和标化DALYs率分别为22.6/10万(95%UI:16.9~29.3/10万)、394.7/10万(95%UI:304.1~494.7/10万),分别较1990年增加了-0.9%和-12.5%。2019年我国归因于肾功能不全的CVD死亡率和DALY率均随年龄增长而上升,并均于95岁以上年龄段达到最高峰;男性归因于肾功能不全的心血管疾病死亡人数、DALYs、年龄标化死亡率和和标化DALY率均高于女性。归因于肾功能不全的心血管疾病主要包括缺血性心脏病、脑出血、缺血性脑卒中和周围血管疾病,其中缺血性心脏病疾病负担最重,DALYs为377.7万人·年(95%UI:243.1~525.3),占2019年归因于肾功能不全的心血管疾病DALYs的50.5%。2019年归因于肾功能不全的缺血性心脏病所致YLLs约为357.8万人·年(95%UI:229.8~499.5),占其所致DALYs的94.7%。结论 与1990年相比,2019年中国归因于肾功能不全的心血管疾病负担上升,且具有性别和年龄差异,其中缺血性心脏病负担最重。

【Abstract】 Objective To review the burden of cardiovascular diseases(CVD) resulted from renal dysfunction in China in 1990 and 2019, and to provide theoretical foundation for cardiovascular disease prevention and treatment.Methods Based on the database of “Global Burden of Disease(GBD) 2019”, the mortality, years of life lost(YLL)and disability-adjusted life years(DALY) of cardiovascular diseases resulted from renal dysfunction were analyzed by using Excel software in population with different ages and genders. Results In 2019, there are 377 thousand cases(95%UI: 285~480) died due to CVD resulted from renal dysfunction, and DALY were 7476 thousand person/year(95%UI: 5797~9391), which increased by 151.3% and 103.8% respectively compared with those in 1990. The agestandardized mortality was 22.6/100 000(95%UI: 16.9~29.3/100 000), and standardized DALY rate was 394.7/100000(95%UI: 304.1~494.7/100 000), which increased by-0.9% and-12.5% respectively compared with those in 1990. The mortality of CVD resulted from renal dysfunction and DALY rate increased gradually with age increased in 2019, and they reached the peak at age more than 95. The died cases due to CVD resulted from renal dysfunction,DALY, age-standardized mortality and standardized DALY rate were higher in male patients than those in female patients. The CVD resulted from renal dysfunction included mainly ischemic heart disease, intracerebral hemorrhage,ischemic stroke and peripheral artery diseases. Among them the burden of ischemic heart disease was the highest, and its DALY was 3777 thousand person/year(95%UI: 2431~5253), which accounted for 50.5% of DALY of CVD resulted from kidney dysfunction in 2019. The YLL of ischemic heart disease induced by kidney dysfunction was approximately 3578 thousand person/year(95%UI: 2298~4995), which accounts for 94.7% of DALY. Conclusion Compared with 1990, the burden of CVD resulted from renal dysfunction increased with difference in gender and age in 2019 in China,and the burden of ischemic heart disease is the highest.
