HMGB1与HMBOX1的高表达在结直肠癌患者预后评估中的作用
Prognostic value of high expression of HMGB1 and HMBOX1 in patients with colorectal cancer
【摘要】 目的:探究高迁移率族蛋白1(HMGB1)与核转录因子1(HMBOX1)的高表达在结直肠癌患者预后评估中的作用。方法:选取96例结直肠癌切除术患者作为研究对象。采用免疫组化法检测各患者结直肠癌组织及远端结直肠组织HMGB1与HMBOX1的表达情况,并根据免疫组化结果不同分为HMGB1高表达组(n=60)与低表达组(n=36),HMBOX1高表达组(n=55)与低表达组(n=41)。分析结直肠癌组织中HMGB1与HMBOX1表达的相关性;采用Kaplan-Meier生存曲线和Log-rank法比较两组患者的生存率;Cox回归综合生存分析影响患者预后的危险因素,分析结直肠癌患者预后差异。结果:HMGB1与HMBOX1在结直肠癌组织中的表达水平较正常的结直肠组织升高(P<0.05)。HMGB1高表达与患者结直肠癌分化程度具有相关性(P<0.05);HMBOX1高表达与患者年龄、结直肠癌TNM分期及分化程度均具有相关性(P<0.05)。结直肠癌组织中HMGB1与HMBOX1高表达具有相关性(P<0.05)。生存分析结果显示,HMGB1高表达组第3、5年的总生存率及无瘤生存率均低于HMGB1低表达组(P<0.05);HMBOX1高表达组第3、5年的总生存率及无瘤生存率均低于HMBOX1低表达组(P<0.05)。单因素分析结果显示,TNM分期为Ⅲ~Ⅳ、T分级为T3~T4、N分级为N2、HMGB1高表达与HMBOX1高表达均为影响结直肠癌患者总生存时间的独立因素(P<0.05);多因素分析结果显示,T分级为T3~T4、N分级为N2与HMBOX1高表达为结直肠癌患者总生存时间的独立预测因子(P<0.05)。结论:HMGB1高表达与患者结直肠癌分化程度相关,HMBOX1高表达与患者年龄、结直肠癌TNM分期及分化程度相关,HMGB1与HMBOX1的高表达具有相关性,其高表达时患者生存时间减少,可作为患者术后预后评估的重要指标。
【Abstract】 Objective: To explore the role of high mobility group box 1(HMGB1) and nuclear transcription factor HMBOX1(homeobox-containing 1,HMBOX1) in the evaluation of the prognosis of patients with colorectal cancer.Methods:A total of 96 patients who received colorectal cancer resection were selected as the research objects.Immunohistochemistry was used to detect the expression of HMGB1 and HMBOX1 in colorectal cancer tissues and distal colorectal tissues of each patient.According to the immunohistochemistry results, they were diviede into HMGB1 high expression group(n=60)and low expression group(n=36),HMBOX1 high expression group(n=55)and low expression group(n=41),the correlation between the expression of HMGB1 and HMBOX1 in colorectal cancer tissues was analyzed by cross-tabulation, Kaplan-meier survival curve and log-rank method were used to compare the survival rate of patients in the two groups.Cox regression comprehensive survival analysis was conducted to analyze the risk factors affecting the prognosis of patients, and to analyze the difference in prognosis of patients with colorectal cancer.Results:The expression levels of HMGB1 and HMBOX1 in colorectal cancer tissues were higher than those in normal colorectal tissues.The high expression of HMGB1 was correlated with the differentiation degree of colorectal cancer(P<0.05).The high expression of HMBOX1 was correlated with age, TNM stage and differentiation degree of colorectal cancer(P<0.05).The HMGB1 was correlated with the high expression of HMBOX1 in colorectal cancer tissues(P<0.05).The results of survival analysis showed that the overall survival rate and disease-free survival rate at 3 and 5 years in the group with high HMGB1 expression were lower than those in the group with low HMGB1 expression.The overall survival rate and disease-free survival rate at 3 and 5 years in high HMBOX1 expression group were lower than those in low HMBOX1 expression group.Univariate analysis showed that TNM stage Ⅲ~Ⅳ,T grade T3~T4,N grade N2,HMGB1 high expression and HMBOX1 high expression were the factors influencing the overall survival time of colorectal cancer patients(P<0.05).Multivariate analysis showed that T grade T3~T4,N grade N2 and high HMBOX1 expression were independent predictors of overall survival time of colorectal cancer patients(P<0.05).Conclusion:The high expression of HMGB1 is significantly correlated with the differentiation degree of colorectal cancer.The high expression of HMBOX1 is significantly correlated with the age, TNM stage and differentiation degree of colorectal cancer.The high expression of HMGB1 is correlated with the high expression of HMBOX1.The survival time of patients with high expression is reduced, which can be used as an important index for postoperative prognosis evaluation.
