益生菌辅助治疗对肝硬化患者肝功能和肠道菌群影响的Meta分析
Meta-analysis of the effects of assisted probiotic therapy on liver function and intestinal flora in patients with cirrhosis
【摘要】 目的 通过整合相关随机对照试验,探讨在常规治疗基础上增加益生菌辅助治疗对肝硬化患者肝功能和肠道菌群的影响,为临床肝硬化的治疗提供参考依据。方法 检索中国生物医学文献服务系统、中国知识基础设施工程、Web of Science、Cochrane Library等数据库中有关采用益生菌辅助治疗肝硬化的文献,检索时间建库至2022年9月30日。2名研究者独立筛选文献并评估偏倚风险,采用RevMan 5.3分析数据。结果 最终纳入48篇文献研究,中文43篇,英文5篇。Meta分析结果显示血浆内毒素:MD=-0.11,95%CI:[-0.13,-0.08];谷丙转氨酶:MD=-9.62,95%CI:[-13.11,-6.12];谷草转氨酶:MD=-9.74,95%CI:[-11.74,-7.75];总胆红素:MD=-6.68,95%CI:[-9.60,-3.75];白蛋白:MD=1.50,95%CI:[0.63,2.36];乳杆菌:MD=2.22,95%CI:[1.17,3.27];双歧杆菌:MD=2.67,95%CI:[1.89,3.46];肠球菌:MD=0.97,95%CI:[0.13,1.80];酵母样真菌:MD=-0.81,95%CI:[-1.35,-0.28]。结论 益生菌治疗能够改善肝硬化患者肝功能,调节肠道菌群,减轻肠道屏障损害。
【Abstract】 Objective Through integrating relevant randomized controlled trials to dicuss the effects of adding probiotic-assisted therapy on liver function and intestinal flora in patients with cirrhosis on the basis of conventional treatment, and to provide evidence for clinical treatment and nursing of cirrhosis. Method Literature related to liver cirrhosis and probiotics was searched in Chinese and foreign databases such as SinoMed,China National Knowledge Infrastructure, Web of Science and the Cochrane Library. The search period was from the establishment of the databases to September 30, 2022. Two researchers independently screened the literature and assessed the risk of bias. The data were analyzed using RevMan 5.3. Results 48 studies were covered, including 43 in Chinese and 5 in English. Meta-analysis results showed that for endotoxin: MD =-0.11, 95% CI: [-0.13,-0.08], alanine aminotransferase: MD =-9.62, 95% CI: [-13.11,-6.12], aspertate aminotransferase: MD =-9.74, 95% CI: [-11.74,-7.75], total bilirubin: MD =-6.68, 95% CI: [-9.60,-3.75], albumin: MD = 1.50, 95% CI: [0.63, 2.36], Lactobacillus: MD = 2.22, 95% CI: [1.17, 3.27], Bifidobacterium: MD =2.67, 95% CI: [1.89, 3.46], Enterococcus: MD = 0.97, 95% CI: [0.13, 1.80], and yeast-like fungi: MD =-0.81,95% CI: [-1.35,-0.28]. Conclusion Probiotics can improve liver function, regulate intestinal flora and reduce intestinal barrier damage in patients with cirrhosis.
