血清CCL11、sIL-2R与糖尿病视网膜病变患者微血管损伤的关系

【摘要】 目的 探讨血清嗜酸性粒细胞趋化因子(CCL11)、可溶性白细胞介素-2受体(sIL-2R)与糖尿病视网膜病变(DR)患者微血管损伤的关系。方法 选取2021年7月—2022年7月收治的122例2型糖尿病作为研究组,根据眼底照相及眼底镜检查结果又分为无视网膜病变亚组(无病变亚组)与视网膜病变亚组(病变亚组);另选同期体检健康志愿者46例作为对照组。采用酶联免疫吸附试验检测血清CCL11、sIL-2R水平。采用受试者工作特征(ROC)曲线评估血清CCL11、sIL-2R对DR患者微血管损伤的预测价值,采用多因素Logistic回归分析探讨DR患者微血管损伤的影响因素。结果 研究组血清sIL-2R、CCL11水平高于对照组(P<0.01)。病变亚组血清sIL-2R、CCL11水平高于无病变亚组(P<0.01)。血清sIL-2R、CCL11预测DR患者微血管损伤的曲线下面积(AUC)分别为0.829、0.765,二者联合预测的AUC为0.903。sIL-2R≥345.84 U/mL、CCL11≥67.98 pg/mL是DR患者微血管损伤的影响因素(P<0.01)。结论 血清sIL-2R、CCL11在DR患者中异常高表达,可能与DR患者视网膜病变密切相关,可将其作为评估DR患者微血管损伤的生物学指标,且二者联合预测效能更高。

【Abstract】 Objective To investigate the relationship between serum eosinophilic chemokine(CCL11), soluble interleukin-2 receptor(sIL-2R) and microvascular injury in patients with diabetic retinopathy(DR). Methods A total of 122 patients with type 2 diabetes treated from July 2021 to July 2022 were selected as the research group. According to the results of fundus photography and fundus microscopy, they were divided into two groups: non-retinopathy subgroup(non-lesion subgroup) and retinopathy subgroup(lesion subgroup). Another 46 healthy volunteers who presented to our hospital for physical examination during the same period were selected as the control group. Serum CCL11 and sIL-2R levels were detected by enzyme-linked immunosorbent assay(ELISA). The predictive value of serum CCL11 and sIL-2R on microvascular injury in DR patients was evaluated by receiver operating characteristic(ROC) curve, and the influencing factors of microvascular injury in DR patients were investigated by multivariate Logistic regression analysis. Results The levels of serum sIL-2R and CCL11 in the research group were higher than those in the control group(P<0.01). The levels of serum sIL-2R and CCL11 in the lesion subgroup were higher than those in the non-lesion subgroup(P<0.01). The area under the curve(AUC) of serum sIL-2R and CCL11 for predicting microvascular injury in DR patients was 0.829 and 0.765, respectively, and the AUC of the combined prediction was 0.903. sIL-2R≥345.84 U/mL and CCL11≥67.98 pg/mL were the influential factors of microvascular injury in DR patients(P<0.01). Conclusion The abnormally high expression of serum sIL-2R and CCL11 in DR patients may be closely related to retinopathy in DR patients, which can be used as biological indicators to evaluate the microvascular injury in DR patients, and the combined prediction efficiency of the two is higher.
