超声自由解剖成像和MRI诊断孕中晚期胎儿颅脑畸形的差异性和一致性对比分析
Comparative Analysis of the Difference and Consistency Between Ultrasound Free Anatomic Imaging and MRI in the Diagnosis of Fetal Craniocerebral Malformation in the Middle and Late Pregnancy
【摘要】 目的 对比分析超声自由解剖成像和MRI诊断孕中晚期胎儿颅脑畸形的差异性和一致性。方法 选取2018年1月—2021年12月170例孕中晚期胎儿颅脑畸形病例,回顾分析其产前超声自由解剖成像和MRI检查情况,统计超声自由解剖成像和MRI检查诊断胎儿颅脑畸形的结果,分析超声自由解剖成像和MRI检查的检出情况,评估超声自由解剖成像和MRI检查获取的图像情况。结果 超声自由解剖成像和MRI检查不同类型胎儿颅脑畸形的诊断结果差异无统计学意义(P>0.05)。MRI检查的诊断准确率明显高于超声自由解剖成像,误诊率及漏诊率低于超声自由解剖成像(P<0.05,P<0.01)。MRI检查敏感度及特异度明显高于超声自由解剖成像(P<0.01)。MRI检查的图像合格率为99.41%(169/170)高于超声自由解剖成像的88.82%(151/170)(P<0.01)。结论 相较于超声自由解剖成像,MRI检查在孕中晚期胎儿颅脑畸形诊断中具有良好的准确率、敏感度、特异度和图像合格率,且误诊率及漏诊率较低,应用效果更佳。但在临床中,医师需根据胎儿实际情况选择合适的诊断方式。
【Abstract】 Objective To compare and analyze the difference and consistency of ultrasound free anatomical imaging and MRI in the diagnosis of fetal craniocerebral malformation in the middle and late pregnancy. Methods From January 2018 to December 2021, 170 patients with fetal craniocerebral malformation in the middle and late pregnancy were selected, and the prenatal ultrasound free anatomical imaging and MRI were retrospectively analyzed. The results of diagnosis of fetal craniocerebral malformation by ultrasound free anatomical imaging and MRI were calculated, and the detection status of ultrasound free anatomical imaging and MRI examination were analyzed. The images obtained by ultrasound free anatomical imaging and MRI were evaluated. Results There was no significant difference in the diagnostic results of different types of fetal craniocerebral malformation between ultrasound free anatomical imaging and MRI(P>0.05). The diagnostic accuracy of MRI was significantly higher than that of ultrasound free anatomical imaging, and the rate of misdiagnosis and missed diagnosis was lower than that of ultrasound free anatomical imaging(P<0.05, P<0.01). The sensitivity and specificity of MRI were significantly higher than that of ultrasound free anatomical imaging(P<0.01). The qualified rate of MRI was 99.41%(169/170) compared with 88.82%(151/170) of ultrasound free anatomical imaging(P<0.01). Conclusion Compared with ultrasound free anatomical imaging, MRI has good accuracy, sensitivity, specificity and image qualification rate in the diagnosis of fetal craniocerebral malformation in the middle and late pregnancy, and the rate of misdiagnosis and missed diagnosis is lower, and the application effect is better. However, in clinical practice, the doctor needs to choose the appropriate diagnostic method according to the actual situation of the fetus.
