浸润性乳腺癌锥光束乳腺CT平扫影像特征与分子亚型及免疫组织化学受体的相关性研究
Study on the Correlation between the Characteristics of Non-Contrast-Enhanced Conebeam Breast CT with Molecular Subtypes and Immunohistochemical Receptors in Invasive Breast Cancer
【摘要】 目的 探讨浸润性乳腺癌锥光束乳腺CT(CBBCT)影像学特征与分子亚型及免疫组织化学受体之间的相关性。方法 回顾性分析129例2020年10月至2021年11月经术后病理证实为浸润性乳腺癌患者的临床病理资料,并且术前均行CBBCT平扫。由两名有多年读片经验的放射科医师用乳腺影像报告和数据系统(BI-RADS)分类标准评估CBBCT图像特征。采用独立样本t检验和卡方检验分析CBBCT特征与分子亚型及受体状态之间的关系,采用Cramer’s V系数评价相关程度,P<0.05为差异具有统计学意义。结果 乳腺癌分子亚型与病变内是否含肿块具有显著相关性(V=0.33,P<0.05),Luminal A型、Luminal B型及三阴性乳腺癌多表现为肿块样病变。乳腺癌分子亚型与病变内是否含钙化具有相关性(V=0.27,P<0.05),三阴性乳腺癌多表现为非钙化样病变。肿块样病变中,Ki-67低增殖组较Ki-67高增殖组肿块边缘多表现为毛刺,肿块形态多表现为不规则;激素受体阳性较激素受体阴性肿块边缘多表现为毛刺,肿块形态多表现为不规则;雌激素受体(ER)阴性肿块[(2.91±0.98)cm]明显大于ER阳性肿块[(2.38±1.27)cm],差异均具有统计学意义(P<0.05)。结论 浸润性乳腺癌CBBCT影像学表现与其分子亚型及免疫组织化学受体具有一定的相关性。
【Abstract】 Objective To investigate the correlation between the imaging characteristics of invasive breast cancer Cone-Beam-Breast CT(CBBCT)and molecular subtypes and immunohistochemical receptors. Methods The clinicopathological data of 129 patients with invasive breast cancer confirmed by postoperative pathology from October 2020 to November 2021 were retrospectively studied, and all patients underwent preoperative non-contrast-enhanced conebeam breast CT.CBBCT image features were evaluated using BI-RADS classification criteria by two radiologists with more than years of experience in reading films.Chi-square test and Independent Samples t-test was used to analyze the relationship between CBBCT characteristics and molecular subtypes and receptor status.Cramer’s V coefficient was used to evaluate the degree of correlation, and P< 0.05 was considered statistically significant. Results There was a significant correlation between molecular subtypes of breast cancer and whether there were masses in the lesions(V=0.33,P< 0.05).Luminal A,Luminal B and Triple negative breast cancer were mostly presented as mass like lesions.The molecular subtypes of breast cancer were correlated with the presence of calcification in the lesions(V=0.27,P< 0.05),and triple negative breast cancer mostly showed non-calcification like lesions.Among the mass-like lesions, the mass margins of low-proliferation Ki-67 group were more burr and the mass morphology was more irregular than that of high-proliferation Ki-67 group.The fringe of the hormone receptor positive mass was more burr and the shape of the mass was irregular than that of the hormone receptor negative mass.ER negative mass(2.91±0.98)cm was significantly higher than ER positive mass(2.38±1.27)cm, with statistical significance(P< 0.05). Conclusion CBBCT imaging features of invasive breast cancer have certain correlation with its molecular subtypes and immunohistochemical receptors.
