40例成人牙周炎患者龈沟液分离厌氧菌分布及其对药物敏感性
Distribution and antimicrobial susceptibility of subgingival anaerobic bacteria isolated from 40 adult patients with periodontitis
【摘要】 目的 了解成人牙周炎患者龈沟液厌氧菌分布及其对临床常用抗菌药物的敏感性。方法 2020年12月—2021年5月收集来自复旦大学附属华山医院、上海市静安区牙病防治所和上海市嘉定区牙病防治所共计40例成人牙周炎患者的龈沟液进行厌氧菌培养和分离,采用基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)进行菌种鉴定;琼脂稀释法测定存活细菌对阿莫西林等12种抗菌药物的体外敏感性。结果 共培养分离到厌氧菌192株,检出率前五位的细菌依次为中间普氏菌、变黑普氏菌、具核梭杆菌、黄褐二氧化碳嗜纤维菌以及黏性放线菌。96株存活厌氧菌对阿莫西林-克拉维酸及美罗培南均敏感。除普氏菌属(敏感率61.3%)外,其余分离菌对阿莫西林亦多呈敏感。普氏菌属及韦荣球菌属对甲硝唑全部敏感,而放线菌属和颗粒链球菌属则对甲硝唑近100%耐药。除了韦荣球菌属外,其他分离菌对多西环素和莫西沙星均敏感。结论 牙周炎患者龈沟液分离厌氧菌以普氏菌属、梭杆菌属和二氧化碳嗜纤维菌属多见;对阿莫西林、阿莫西林-克拉维酸、多西环素和甲硝唑多呈敏感。
【Abstract】 Objective To investigate the distribution and antimicrobial susceptibility of anaerobic strains isolated from gingival crevicular fluid of patients with periodontitis. Methods Gingival crevicular fluid was collected from 40 adults with periodontitis in Huashan Hospital, Fudan University, Jing’an District Dental Institute, and Jiading District Dental Institute from December 2020 to May 2021. Anaerobic bacteria were cultured with the specimens and identified by matrix-assisted laser desorption/ionization timeof-flight mass spectrometry(MALDI-TOF MS). The susceptibility of the isolates to 12 antimicrobial agents was determined by agar dilution method. Results A total of 192 anaerobic bacteria were isolated. The top five species were Prevotella intermedia(65.0%),Prevotella nigrescens(55.0%), Fusobacterium nucleatum(52.5%), Capnocytophaga ochracea(37.5%), and Actinomyces viscosus(30.0%). Overall, 96 isolates were successfully revived, all of which were susceptible to amoxicillin-clavulanic acid and meropenem.Almost all isolates except Prevotella spp.(61.3% susceptible) were susceptible to amoxicillin. Prevotella spp. and Veillonella spp. were all susceptible to metronidazole, while Actinomyces spp. and Granulicatella spp. were nearly 100% resistant to metronidazole. All other isolates except Veillonella spp. were susceptible to doxycycline and moxifloxacin. Conclusions Prevotella spp., Fusobacterium spp. and Capnocytophaga spp. were the prevalent anaerobic bacteria in the gingival crevicular fluid of periodontitis patients. Most of the isolates were susceptible to amoxicillin, amoxicillinclavulanic acid, doxycycline, and metronidazole.
