早产儿支气管肺发育不良及其严重程度的危险因素分析
Analysis of risk factors for bronchopulmonary dysplasia and its severity in premature infants
【摘要】 目的:探讨早产儿支气管肺发育不良(BPD)及其严重程度的危险因素。方法:收集胎龄≤34周早产儿175例,分为BPD组、非BPD组、轻度BPD组及中重度BPD组,回顾性研究BPD及BPD严重程度的危险因素,并通过Logistic回归分析探讨其独立危险因素。结果:子痫前期、宫内窘迫、胎盘早剥、双胎儿、前置胎盘、产前激素应用对BPD发生无显著影响,孕母胎膜早破是发生BPD的独立危险因素。入院1周时热量供给、肠外营养时间、机械通气时间、早产儿呼吸暂停(AOP)、新生儿肺炎、新生儿败血症、1分钟Apgar评分≤7是BPD发生的危险因素。机械通气时间是发生中重度BPD的独立危险因素。结论:孕母胎膜早破、早产儿呼吸暂停、新生儿窒息及感染、1周时热量供应不足等可致BPD风险增加;早产儿肠外营养时间及机械通气时间越长,发生BPD风险越高;早产儿机械通气时间越长,越易发生中重度BPD。
【Abstract】 Objective: To explore the risk factors of bronchopulmonary dysplasia(BPD) and its severity in premature infants. Methods: 175 premature infants with gestational age ≤ 34 weeks were collected and divided into BPD group, non BPD group, mild BPD group, moderate and severe BPD group. The risk factors of BPD and the severity of BPD were studied retrospectively, and their independent risk factors were discussed by logistic regression analysis. Results: Preeclampsia, intrauterine distress, placental abruption, twins, placenta previa, and prenatal hormone use had no significant effect on the occurrence of BPD. Premature rupture of membranes was an independent risk factor for BPD.Heat supply(calories) at 1 week of admission, parenteral nutrition time, mechanical ventilation time, premature apnea(AOP), neonatal pneumonia, neonatal sepsis and 1-minute Apgar score ≤ 7 were independent risk factors for BPD. Mechanical ventilation time was an independent risk factor for moderate to severe BPD. Conclusion: The premature rupture of membranes, apnea of premature infants, neonatal asphyxia and infection, insufficient heat supply at one week, which can increase the risk of BPD; The longer time of parenteral nutrition and mechanical ventilation, the higher risk of BPD. The longer mechanical ventilation time of premature infants, the more likely they are to develop moderate to severe BPD.
