【摘要】 目的 研究和验证红藤灌肠方联合CT引导下介入治疗妇科恶性肿瘤患者术后盆腔淋巴囊肿的有效性和安全性。方法 选取2021年1月至2022年10月在我院行妇科恶性肿瘤根治手术（术式中包含系统性盆腔淋巴结清扫或腹主动脉旁淋巴结切除者）后出现症状性盆腔淋巴囊肿的患者共40例纳入研究。将患者随机选入对照组与实验组，各20例。对照组应用16排螺旋CT及8Fr PTCD穿刺套件行常规CT引导下介入治疗，实验组在对照组治疗基础上给予自拟红藤灌肠方行保留灌肠治疗。观察患者治疗前和治疗14天后中医症候积分变化，比较两组治疗后临床疗效相关指标并计算两组不良反应发生率。结果 实验组的治疗后中医证候总积分显著低于对照组，差异有统计学意义（P<0.05），实验组治疗前后的积分变化明显优于对照组，差异有统计学意义（P<0.05）。实验组的治疗总有效率明显高于对照组，且治愈人数显著高于对照组，差异有统计学意义（P<0.05）。实验组的置管时间明显短于对照组，差异有统计学意义（P<0.05）。然而，在囊肿完全消退时间及复发人数方面，两组差异无统计学意义（P>0.05）。两组总体副反应和并发症发生率差异无统计学意义（P>0.05）。结论 红藤灌肠方联合CT引导下介入治疗妇科恶性肿瘤患者术后盆腔淋巴囊肿是一套实施简便、疗效确切、安全可靠、副作用小的中西医结合精准治疗方案。更多还原

【Abstract】 Objective To verify the efficacy and safety of Hongteng enema combined with CT-guided interventional therapy for postoperative pelvic lymphocyst of gynecological malignant tumor. Methods A total of 40 patients with symptomatic pelvic lymph cyst after radical surgery for gynecological malignant tumors( including systematic pelvic lymph node dissection or paraaortic abdominal lymph node resection) in our hospital from January 2021 to October 2022 were selected as the research objects. Patients were randomly divided into control group and study group, 20 cases in each group. The control group was treated with 16-slice spiral CT and 18G PTC puncture needle for routine CT-guided interventional therapy, and the study group was treated with self-made Hongteng enema prescription for retention enema on the basis of the treatment of the control group. The changes of TCM symptom scores before treatment and 14 days after treatment were observed, the clinical efficacy of the two groups after treatment was compared, and the incidence of adverse reactions in the two groups was calculated. Results After treatment, the total TCM symptom scores in the experimental group was obviously lower than that in the control group, with a statistically significant difference(P<0.05), the score changes before and after treatment were significantly better in the experimental group than those in the control group, with a statistically significant difference(P<0.05). The total effective rate of treatment in the experimental group was obviously higher than that in the control group, and the number of cured patients was significantly more than that in the control group, with a statistically significant difference(P<0.05). The catheterization time in the experimental group was significantly shorter than that in the control group, with a statistically significant difference(P<0.05). However, statistically significant difference between the two groups was not observed in the time of complete regression of cysts and the number of recurrences(P>0.05). There was no statistically significant difference in overall side effects and complications between the two groups(P>0.05). Conclusion Hongteng enema prescription combined with CT-guided interventional treatment of postoperative pelvic lymphocyst of gynecological malignant tumor is a set of accurate treatment scheme of integrated traditional Chinese and western medicine with simple implementation, exact curative effect, safe and reliable, and small side effects.更多还原
