儿童青少年血压与肥胖关联性的分位数回归分析
Quantile regression analysis of association between blood pressure and obesity among children and adolescents
【摘要】 目的 探讨不同水平血压与体质量指数(BMI)、腰围(WC)、腰高比(WHtR)等肥胖指标的关联性。方法 采用分层整群抽样方法,抽取7~17岁苏州市儿童青少年,采集性别、年龄、身高、体重、腰围、血压等指标,采用分位数回归分析血压值在P5、P10、P25、P50、P75、P90、P95分位数下,肥胖指标与血压之间的关联性。结果 共纳入研究3 150人,男生占50.86%,年龄(12.03±3.12)岁,基于BMI的全身性肥胖检出率为13.24%,基于WC和WHtR的中心性肥胖检出率分别为20.60%和23.65%,血压偏高检出率为11.27%。基于BMI、WC、WHtR分组的血压值,均为超重/肥胖组>正常组(P值均<0.05)。分位数回归结果显示,不同分位点下SBP均与BMI、WC、WHtR呈正相关(P值均<0.001);低DBP(P5)总人群、男生、女生的BMI、WC、WHtR的变化,低DBP(P10)下女生BMI的变化,与DBP均无统计学关联(P值均>0.05)。进一步比较不同血压分位数下BMI、WC、WHtR与血压的关联强度结果显示:在男生和总人群中,与SBP、DBP的关联强度差异均有统计学意义(P值均<0.01);在女生中,不同SBP分位数下,BMI、WC、WHtR与SBP的关联差异无统计学意义(P值均>0.05),不同DBP分位数下,BMI与DBP的关联差异有统计学意义(P=0.028),WC、WHtR与DBP的关联差异无统计学意义(P值均>0.05)。结论 儿童青少年BMI、WC、WHtR与血压呈正相关,该关联性随血压分位点的增加而增强,建议在血压的管理中加强肥胖控制。
【Abstract】 Objective To examine the associations of blood pressure with body mass index(BMI),waist circumstance(WC) and waist-to-height ratio(WHtR). Methods Children and adolescents at ages of 7 to 17 years were sampled using the stratified cluster sampling method in Suzhou City, and participant’ gender, age, height, weight, WC and blood pressure were collected.Quantile regression was used to analyze the associations of BMI,WC and WHtR with blood pressure at different percentiles(P5,P10,P25,P50,P75,P90 and P95). Results A total of 3 150 children and adolescents were enrolled, including 50.86% men and with a mean age of 12.03±3.12 years.The detection of generalized obesity was 13.24% based on BMI,and the detection rates of central obesity were 20.60% and 23.65% based on WC and WHtR,respectively.The detection of elevated blood pressure was 11.27%,and the highest blood pressure was measured in the overweight/obesity group than in the normal group(all P<0.05).Quantile regression analysis showed that systolic blood pressure(SBP) at different percentiles correlated positively with BMI,WC and WHtR(P<0.001),and changes in BMI,WC and WHtR among all participants at P5 diastolic blood pressure(DBP) and among male and female participants at P5 DBP,and BMI changes at P10 DBP among women showed no statistical associations with DBP(all P>0.05).In addition, there were significant differences in the intensity of the association of BMI,WC and WHtR with SBP and DBP at different percentiles among males and all participants(all P<0.001).BMI,WC or WHtR showed no statistical association with SBP at different SBP percentiles among women(all P>0.05),and BMI significantly correlated with DBP at different DBP percentiles(P = 0.028),while WC or WHtR had no statistical association with DBP at different SBP percentiles among women(all P>0.05). Conclusions BMI,WC and WHtR correlate positively with blood pressure among children and adolescents, and the intensity of correlation increases with blood pressure percentiles.Obesity control is recommended to be improved during blood pressure management.
