呼吸肌训练用于重症监护病房机械通气获得性肌无力患者的临床观察
Clinical observation of respiratory training for patients with mechanical ventilation acquired myasthenia gravis in ICU
【摘要】 目的 探讨呼吸肌训练对重症监护病房(ICU)机械通气获得性肌无力患者的临床干预作用。方法 前瞻选取2019年1月~2021年12月新疆维吾尔自治区人民医院收治的ICU机械通气获得性肌无力患者120例,采用随机数表法分为观察组(60例)和对照组(60例),对照组采用ICU常规干预,观察组在对照组基础上采用呼吸肌训练,比较两组机械通气时间、ICU住院时间、总住院时间、脱机拔管率、并发症发生率,对比两组干预前后第1秒用力呼气容积(FEVl)、用力肺活量(FVC)、FEVl/FVC、肌力评定量表(MRC)评分、Barthel指数(MBI)评分、生活质量量表(SF-36)评分。结果 两组脱机拔管率相比,差异无统计学意义(P> 0.05)。观察组机械通气时间、ICU住院时间、总住院时间均明显低于对照组,差异有统计学意义(P <0.05);干预后观察组FEV1、FVC、FEV1/FVC明显高于干预前及对照组干预后,差异有统计学意义(P <0.05);干预后观察组MRC、MBI、SF-36评分均高于干预前及对照组干预后,差异有统计学意义(P <0.05);观察组并发症发生率低于对照组,,差异有统计学意义(P <0.05)。结论 呼吸肌训练干预能明显提高ICU机械通气获得性肌无力患者肺功能,缩短患者机械通气时间、住院时间,明显改善患者肌力水平及日常活动能力,减少并发症发生率,提高患者生活质量。
【Abstract】 Objective To explore the clinical intervention effect of respiratory muscle training on patients with mechanical ventilation acquired myasthenia gravis in intensive care unit(ICU).Methods A total of 120 patients with myasthenia gravis acquired by mechanical ventilation in ICU from January 2019 to December 2021 were selected as the research object. They were divided into observation group(n=60)and control group(n=60)by random number table method. The control group received routine intervention in ICU, while the observation group received additional respiratory muscle training. The two groups were compared in terms of mechanical ventilation time, hospitalization time in ICU, total hospitalization time, off-line extubation rate and complication rate. The first second forced expiratory volume(FEVl),forced vital capacity(FVC),FEVl/FVC,muscle strength rating scale(MRC),Barthel index(MBI)and quality of life scale(SF-36)were compared between the two groups before and after the intervention.Results There was no significant difference in extubation rate between the two groups(P > 0.05).The observation group showed significant shorter mechanical ventilation time, ICU stay duration and total hospitalization time compared to the control group(P < 0.05).FEV1,FVC and FEV1/FVC in the observation group after intervention were significantly higher than those before and after intervention in the control group(P < 0.05).After the intervention,the MRC,MBI and SF-36 scores in the observation group were higher than those before and after the intervention in the control group(P < 0.05).The incidence of complications in the observation group was lower than that in the control group(P < 0.05).Conclusion Respiratory muscle training intervention in ICU patients with acquired myasthenia gravis can significantly improve lung function, reduce their mechanical ventilation time and hospitalization time. It also significantly improve muscle strength and daily activities, decrease complications rates and enhance their quality of life.
