【摘要】 目的 探讨胫骨平台骨折(FTP)患者采用膝关节镜下球囊成形术治疗的效果及对术后关节功能恢复的影响。方法 选取2021年2月至2022年8月信阳职业技术学院附属医院骨科采用膝关节镜下球囊成形术治疗的FTP患者48例作为微创组，同期采用传统切开手术治疗的患者60例作为对照组；统计分析两组的手术时间、手术出血量、完全负重时间等指标，比较术后即刻、术后3个月、术后6个月患者膝关节胫骨平台内翻角(TPA)、后倾角(CA)及膝关节功能。结果 两组患者手术时间、X线曝光时间、手术出血量、术后下床活动时间、术后引流量比较，微创组均低于对照组(P<0.05);微创组和对照组的完全负重时间差异无统计学意义(P>0.05);在术后即刻、术后3个月、术后6个月，微创组患者的膝关节的TPA、CA测定值与对照组比较，差异均无统计学意义(P>0.05);在术后6个月，微创组患者的绞锁、上楼、肿胀、疼痛及Lysholm总评分均高于对照组(P<0.05);微创组患者的跛行、支持、不稳定、下蹲评分与对照组比较，差异无统计学意义(P>0.05)。结论 膝关节镜下球囊成形术治疗的FTP患者在保证骨折复位的同时，减轻手术过程的创伤程度，有利于术后膝关节功能的恢复。更多还原

【Abstract】 Objective To investigate the effect of arthroscopic balloon arthroplasty on patients with tibial plateau fracture(FTP) and its influence on postoperative joint function recovery.Methods A total of 48 FTP patients treated with arthroscopic balloon arthroplasty from February 2021 to August 2022, Orthopedics Department of Affiliated Hospital of Xinyang Vocational and Technical College were selected as the minimally invasive group and 60 patients treated with traditional incision surgery in the same period were selected as the control group. The indexes of operation time, operation blood loss and full weight-bearing time of the two groups were statistically analyzed. The tibial plateau varus angle(TPA), caster angle(CA) and knee function were compared immediately, 3 months and 6 months after surgery.Results The operation time, the time of X-ray exposure, the amount of surgical bleeding, the time of postoperative ambulation, and the postoperative drainage were lower in the minimally invasive group than in the control group(P<0.05). There was no significant difference in full weight-bearing time between the minimally invasive group and the control group(P>0.05). There was no significant difference in the TPA and CA between the minimally invasive group and the control group immediately after surgery, 3 months after operation and 6 months after operation(P>0.05). At 6 months after operation, the lock, upstairs, swelling, pain and Lysholm total scores of the minimally invasive group were higher than those the control group(P<0.05). There was no significant difference in the claudication, support, instability, and squat scores between the minimally invasive group and the control group(P>0.05).Conclusion FTP patients treated with knee arthroscopic balloon angioplasty can reduce the degree of trauma during the operation and ensure the recovery of postoperative knee function while ensuring fracture reduction.更多还原
